Nursing care for patient with total hip arthoplasty

Abstract:

Cílem bakalářské práce je stanovení obecného ošetřovatelského plánu pro pacienta s totální endoprotézou kyčelního kloubu v perioperačním období. Totální náhrada kyčelního kloubu se využívá jako chirurgická metoda léčby kloubních onemocnění. Nejčastějším kloubním onemocněním, postihujícím velkou část populace, je osteoartróza. Tato bakalářská práce shrnuje celkovou problematiku implantace totální endoprotézy kyčelního kloubu v rámci perioperační ošetřovatelské péče a skládá se ze dvou částí, tedy teoretické a praktické části. V teoretické části je stručně zmíněna historie endoprotetiky, jsou zde uvedeny základní typy kyčelních náhrad a nejčastější indikace k provedení tohoto operačního výkonu. Velká pozornost je věnována především osteoartróze, její etiopatogenezi, klinickému obrazu, diagnostice a terapii. V teoretické části jsou také uvedena jednotlivá specifika daného operačního výkonu, tedy technika operace, možnosti kloubní drenáže, autotransfuzní program, rehabilitace a také je zde zmíněna významnost tromboprofylaxe a antimikrobiální profylaxe. Praktická část je zaměřena na tvorbu obecného plánu ošetřovatelské péče. Jednotlivé ošetřovatelské diagnózy jsou stanoveny dle NANDA International Taxonomie II a uspořádány dle jednotlivých fází perioperační ošetřovatelské péče. Ke každé ošetřovatelské diagnóze jsou pak uvedeny patřičné ošetřovatelské cíle a intervence, vedoucí k optimální saturaci potřeb pacienta. Stručně jsou pak nastíněna možná témata edukace.

Abstract:

The aim of my bachelor?s thesis is the setting of the general nursing care plan for a patient with total hip arthroplasty in the perioperative period. Total hip replacement is used as a method of cure of the joint disorder. The most frequent illness that affects the majority of the population is osteoarthritis. This bachelor?s thesis sums up the whole problematic of implementation of the total hip arthroplasty in the perioperative period of nursing care plan and it consists of two parts, theoretical and practical. The history of endoprothetics is briefly mentioned in the theoretical part. There are presented basic types of hip replacements and the most common indications for the execution of the operation. Large attention is given to osteoarthritis, it?s etiopathogenesis, clinical manifestation, diagnosis and therapy. There are each specifics for given operation, technique of the operation, possibilities of joint drainage, autotransfusion program, rehabilitation also stated in the theoretical part. The importance of the tromboprophylaxis and antimicrobial prophylaxis is also presented. The practical part is aimed on the making of the general plan of nursing care. Each nursing diagnosis is set according to NANDA International Taxonomy II and placed according to each phase of perioperative nursing care. Appropriate nursing outcomes and interventions are presented to each diagnosis which lead to the optimal saturation of the patient?s needs. Possible topics of education are briefly presented.
