The effect of monitoring the depth of general anesthesia on the incidence of PONV and developing delirium in children undergoing endoscopic adenotomy under general anesthesia.

Abstract:

Diplomová práce hodnotí vliv monitorace a optimalizace hloubky anestezie prostřednictvím bispektrálního indexu (BIS) na výskyt pooperační nauzey a zvracení (PONV) a rozvoje emergentního deliria (ED) u dětí podstupujících endoskopickou adenotomii v celkové anestezii. Teoretická část práce je tvořena souhrnem publikovaných dat z odborných českých a zahraničních zdrojů zaměřených na problematiku PONV a pooperačního rozvoje emergentního deliria u pediatrických pacientů. Teoretická východiska se dále zabývají celkovou anestezií v pediatrii a rolí bolesti ve vztahu k ED a PONV. Praktická část prezentuje analýzu a grafické znázornění získaných dat prostřednictvím prospektivní randomizované klinické studie. Výzkumný soubor tvořilo 33 pediatrických pacientů ve věku 1-9 let podstupujících endoskopickou adenotomii v celkové anestezii ve Fakultní nemocnici Ostrava v období června 2021 až ledna 2022. Pacienti byli randomizováni při příjezdu na operační sál do intervenčního ramene (BIS monitorace hloubky celkové anestezie, n=20) a do kontrolní skupiny (standardní vedení celkové anestezie k MAC 1,0; n=13). Incidence časné PONV a ED byla hodnocena v průběhu prvních 60 minut na PACU (Post-Anesthesia Care Unit). Pozdní výskyt PONV byl hodnocen na ORL klinice, tj. během 24 hodin po operačním výkonu. K zhodnocení byly využity verifikované škály BARF a PAED skóre. Získaná data byla statisticky zpracována a verifikována za pomocí chí-kvadrát testu nezávislosti v kontingenční tabulce, Fisherova přesného testu, Welchova t-testu a Mann-Whitneyho testu. Statistické testy byly hodnoceny hladinou významnosti 5 %.

Abstract:

The diploma thesis evaluates the influence of monitoring and optimization of the depth of anesthesia through the bispectral index (BIS) on incidence of postoperative nausea and vomiting (PONV) and the development of emergent delirium (ED) in children undergoing endoscopic adenotomy under general anesthesia. The theoretical part of the work consists of summary of published data from professional Czech and foreign sources focused on the issue of PONV and postoperative development of emergence delirium (ED) in pediatric patients. Theoretical background also deals with general anesthesia in pediatrics and the role of pain in relation to ED and PONV. The practical part presents an analysis and graphical representation of the data obtained from prospective randomized clinical trial. The research group consisted of 33 pediatric patients aged 1-9 years undergoing endoscopic adenotomy under general anesthesia at the University Hospital Ostrava in the period from June 2021 to January 2022. Patients were randomized upon arrival in the operating room to the interventional arm (BIS monitoring of general anesthesia, n=20) and to the control group (standard management of general anesthesia to MAC 1,0, n=13) The incidence of early PONV and ED was assessed during the first 60 minutes at the PACU (Post-Anesthesia Care Unit). The late incidence of PONV was evaluated at ENT clinic, within 24 hours after surgery. Verified scales were used for evaluation: BARF and PAED scores. The data obtained were statistically processed and verified using the chi-square PivodTable independence test, Fisher's exact test, Welch's t-test and Mann-Whitney test. Statistical tests were evaluated with a significance level of 5 %.
