The use of joinpoint regression to predict the development of oncological diseases

Abstract:

Tato rigorózní práce se zabývá využitím metody joinpoint regrese v predikci vývoje onkologických onemocnění. Prediktivní modely v epidemiologii nádorových onemocnění jsou nedílnou součástí plánování a organizace zdravotní péče. Na základě prediktivních modelů je například možné odhadnout očekávané náklady spojené s nově diagnostikovanými případy, což vede ke správnému rozvržení finančních zdrojů potřebných pro budoucí diagnostiku a léčbu daného onemocnění. Přínos joinpoint regrese v predikčních modelech spočívá ve spolehlivé identifikaci vhodného časového okna, které je pro predikci využíváno. Časové okno nalezené pomocí metody joinpoint regrese odráží aktuální změny ve vývoji daného onemocnění, což minimalizuje riziko, že predikční model bude založen na historických trendech, které již nejsou pro současnou situaci aktuální. V předložené rigorózní práci je detailně popsána metoda joinpoint regrese včetně různých přístupů možného využití této regresní metody v predikci budoucího vývoje daného onemocnění. V rámci praktické části rigorózní práce byly vybrané prediktivní modely aplikovány na data Národního onkologického registru a byla provedena analýza šesti onkologických diagnóz. Cílem praktické části bylo srovnání různých predikčních modelů využívajících metodu joinpoint regrese a následně provedení predikce vývoje incidence pro období 2017–2020 pomocí nejvhodnějšího z uvažovaných modelů. Celkem bylo srovnáno šest různých predikčních modelů, přičemž jako nejvhodnější se ukázal lineární regresní model vytvořený pro období odpovídající poslednímu segmentu joinpoint modelu určeného na základě permutačního testování. Následná predikce pomocí tohoto modelu předpovídá rostoucí vývoj incidence v letech 2017–2020 pro všechny analyzované diagnózy. Největší nárůst incidence je predikován pro non-Hodgkinův lymfom (procentuální změna za období 2017–2020: 2,0 %), naopak nejmírnější růst je předpokládaný pro zhoubné novotvary tlustého střeva a konečníku (procentuální změna za období 2017–2020: 0,1 %). Výsledky z provedené analýzy potvrdily celosvětově predikovaný budoucí růst onkologických onemocnění související především s významným demografickým trendem současné doby, stárnutím populace.

Abstract:

This rigorosum thesis deals with the use of joinpoint regression to predict the development of oncological diseases. In the field of epidemiology of oncological diseases, predictive models are an indispensable part of planning and organization of health care. For instance, they help estimate expected costs of newly diagnosed cases, leading to appropriate allocation of financial resources for future diagnostics and treatment of a disease. The contribution of joinpoint regression to predictive models is a reliable identification of a suitable time window that is used for prediction. The time window determined by the joinpoint regression method reflects current changes in the development of a disease, minimizing the risk of the prediction model being based on historical trends that are no longer relevant to the current situation. The presented thesis introduces joinpoint regression in detail including different approaches to possible uses of the method in predicting the future development of a particular disease. In the practical part of the thesis, the predictive models are applied to the data of the Czech National Cancer Registry and an analysis of six oncological diagnoses is carried out. The aim of this part is to compare various predictive models based on the joinpoint regression method and subsequently estimate the development of incidence for the period of 2017–2020 by means of the most suitable models. In total, six different predictive models were compared, with a linear regression model based on the period corresponding to the last segment of the joinpoint model determined with permutation testing performing the best. The subsequent prediction using this model shows increasing development of incidence in 2017–2020 for all analyzed diagnoses. The largest increase being predicted is for non Hodgkin’s lymphoma (2.0% percentage change in 2017–2020), whereas the slowest growth is predicted for malignant neoplasms of colon and rectum (0.1% percentage change in 2017–2020). The results of the analysis confirmed the worldwide predicted future growth of oncological diseases mainly related to the significant demographic trend of the present time, the aging of the population.
