Abstract:

Primárním cílem této disertační práce bylo shrnout současný stav literatury o negativních účincích psychoterapie a poskytnout nové poznatky, které požadovali jiní výzkumní autoři. V rámci disertační práce byly provedeny celkově tři výzkumné studie s cílem navázat na předcházející autory v dané oblasti a rozvinout jejich úvahy a zjištění. V první studii jsme ověřovali, že česká verze dotazníku NEQ pro měření negativních účinků psychoterapie můžeme považovat za validní a spolehlivý nástroj. Cílem první studie bylo adaptovat dotazník negativních účinků psychoterapie (NEQ) do českého kontextu a posoudit jeho psychometrické vlastnosti a také rozsah, v jakém pokrývá důležité oblasti negativních aspektů psychoterapie. K analýze otevřené položky the NEQ, co jiné negativní respondenti v psychoterapii zažili byla použita tematická analýza. Ve výsledcích jsme zjistili, že šestifaktorový model navržený autory NEQ neprokázal adekvátní fit. V následné explorační faktorové analýze bylo místo toho navrženo vhodnější pětifaktorové řešení. Tematická analýza navíc odhalila některé oblasti nedostatečně zastoupené v NEQ, zejména vztahy klientů s jejich příbuznými. Obecně lze po první studii říct, že česká verze NEQ je spolehlivým nástrojem s vysokou přidanou hodnotou pro výzkum i praxi. Cílem druhé studie bylo popsat mnoho zkušeností pana L s několika psychoterapiemi v průběhu 15 let. K prezentaci zkušeností pana L s psychoterapiemi jsme použili induktivní tematickou analýzu. Našli jsme mnoho témat, která byla kategorizována do tří hlavních skupin jako 1. negativní zkušenosti, 2. pocity nekompaktnosti, nevhodných metod a stylů léčby a 3. negativní důsledky terapií. Navrhli jsme vysvětlující model, který shrnuje, jak mohou být zkušenosti pana L propojeny. Zkušenosti pana L s psychoterapiemi v průběhu 15 let byly spojeny se silnými negativními pocity, myšlenkami a přetrvávajícími následky. Navrhli jsme několik možných metod, jak těmto jevům předcházet nebo je řešit. Na závěr, cílem poslední studie bylo prozkoumat možné vztahy mezi osobnostními rysy HEXACO (tj. Extraverze, Emocionalita, Upřímnost-Pokora, Přívětivost, Svědomitost, Otevřenost vůči zkušenosti) a negativními účinky psychoterapie. V online průzkumu odpovědělo celkem 148 respondentů. Byly provedeny deskriptivní a multivariační regresní analýzy. Ve výsledcích jsme zjistili, že Extraverze byla nejsilnějším negativním prediktorem negativních účinků a sloužila jako ochranný faktor proti zhoršování symptomů, beznaději a stigmatizaci. Dále Emocionalita souvisela s nižší vnímanou kvalitou psychoterapie, Svědomitost souvisela s beznadějí a Otevřenost prožitku sloužila jako ochránce před závislostí. Zjištění jsou založena na jednorázovém retrospektivním měření a na malém vzorku. Výsledky je proto nutné interpretovat opatrně, protože předpoklady lineární regrese nebyly splněny. Výsledky všech tří studií nejsou schopny odpovědět na všechny potřebné výzkumné otázky. Budoucí výzkum by se měl zaměřit na sjednocení terminologie, definice a ověření potenciálních faktorů přispívajících k negativním účinkům psychoterapie.

Abstract:

The primary goal of this dissertation was to summarize the current state of the literature on the negative effects of psychotherapy, and to provide new knowledge that was requested previously by other authors. A total of three research studies were developed as part of the dissertation, expanding on the work of previous authors in the given area, to supplement and develop their considerations and findings. The aim of the first study was to adapt the Negative Effects Questionnaire (NEQ) into the Czech context and to assess its psychometric properties as well as the extent to which it covers important areas of the negative aspects of psychotherapy. Thematic analysis was used to analyse the open-ended NEQ item. In results, we found that the six-factor model proposed by the authors of the NEQ did not demonstrate an adequate fit. In a subsequent exploratory factor analysis, a more parsimonious, five-factor solution was proposed instead. Furthermore, a thematic analysis revealed some areas underrepresented in the NEQ, particularly clients’ relationships with their relatives. Generally, after the first study we can say that the Czech version of the NEQ is a reliable instrument with a high added value for research and practice. The aim of the second study was to describe the many experiences of Mr. L with several psychotherapies over a 15-year period. To present the experiences of Mr. L with psychotherapies, we used inductive thematic analysis. We found many themes that were categorized under three major clusters as 1. negative experiences, 2. feelings of unfitness and inappropriate methods and styles of treatment, and 3. negative consequences of the therapies. We proposed an explanatory model to summarize how the experiences of Mr. L might be connected. The experiences of Mr. L with psychotherapies over the course of 15 years were associated with strong negative feelings, thoughts, and ongoing consequences. We proposed several possible methods for avoiding or addressing these phenomena. Lastly, the aim of the last study was to explore possible relationships between the HEXACO personality traits (i.e., Extraversion, Emotionality, Honesty-Humility, Agreeableness, Conscientiousness, Openness to experience) and negative effects of psychotherapy. A total of 148 respondents answered an online survey. Descriptive and multivariate regression analyses were performed. In results we found that Extraversion was the strongest negative predictor of negative effects, serving as a protective factor against worsening of symptoms, hopelessness, and stigma. Furthermore, Emotionality was related to lower perceived quality of psychotherapy, Conscientiousness was related to hopelessness, and Openness to experience served as a protector against dependency. The findings are based on a onetime retrospective measurement and a convenience sample. Therefore, the results must be interpreted with caution because the assumptions of linear regression were not fulfilled. The results of all three studies are unable to answer to all needed research questions. Future research should aim on unifying terminology, definition and to verify potential factors contributing to the negative effects of psychotherapy.
