Somatosensory rehabilitation in patients with peripheral vestibular syndrome

Abstract:

Tato diplomová práce je zaměřena na problematiku periferního vestibulárního systému. Hodnotí účinek každodenní terapie zaměřené na somatosenzorický systém, která probíhala po dobu 8 týdnů. Hlavním cílem práce je posoudit efekt zvolené terapie na posturální stabilitu a kvalitu života u pacientů s periferní vestibulární lézí. Výzkumu se zúčastnilo 11 pacientů se závratěmi způsobené periferní vestibulární lézí ve věku 51,3 ? 8,2 let, s délkou trvání potíží od 3 po 60 měsíců. V prvních pěti týdnech docházeli pacienti dvakrát týdně na oddělení léčebné rehabilitace, kde absolvovali celkem 10 terapeutických intervencí pod vedením fyzioterapeuta, ostatní dny prováděli cvičební jednotku samostatně v domácím prostředí. Zbylé tři týdny byly vedeny formou každodenní autoterapie a domácího cvičení. Výzkumná část se zabývala porovnáním parametrů průměrných rychlostí výchylek COP v mediolaterálním směru v pěti typech stoje před a po terapeutické intervenci. Pro hodnocení rychlosti výchylek COP byla využita silová plošina Kistler. Pro hodnocení subjektivního vnímání závratí a kvality života byl využit dotazník Dizzines Handicap Inventory (DHI). Ve výsledku byl statisticky významný rozdíl v průměrné rychlosti výchylek COP mediolaterálním směrem nalezen pouze u stoje s vyloučením zrakové fixace. V ostatních typech stoje vytvořená fyzioterapeutická intervence neměla významný statistický vliv na posturální stabilitu pacientů. S výjimkou stoje na pravé dolní končetině však byly zaznamenány tendence snižování mediánu průměrných rychlostí výchylek COP v mediolaterálním směru. Z hlediska subjektivního vnímání závratí a kvality života pacientů se závratěmi bylo pomocí DHI dotazníku prokázáno signifikantní zlepšení.

Abstract:

This diploma thesis si focused on the issue of peripheral vestibular system. It evaluates the effect of a daily therapy focused on the somatosensory system, which lasted over a period of 8 weeks. The main aim of the work was to assess the effect of the chosen therapy on postural stability and quality of life in the group of patients with peripheral vestibular lesions. Eleven patients with vertigo caused by a peripheral vestibular lesion were participated in the research aged around 51.3 ? 8.2 years, with a duration of problems from 3 to 60 months. In the first five weeks, the patients attended the medical rehabilitation department twice a week, where they completed 10 therapeutic interventions under the guidance of a physiotherapist, the rest of the days they performed exercises independently at home. The remaining three weeks were guided in the form of daily self-therapy and home exercises. The practical part was focused on the comparison of the average parameters of COP deflection speeds in the mediolateral direction in five types of standing before and after the therapeutic intervention. The Kistler force platform was used to evaluate the COP deflection speeds. The Dizzines Handicap Inventory (DHI) questionnaire was used to assess the subjective perception of dizziness and quality of life. The research has found a statistically significant difference in the average of COP deflection speeds in the mediolateral direction just in the standing with exclusion of visual fixation. The group of other types of standing did not have a statistically significant effect on the postural stability of the patient affected by physiotherapeutic intervention. However, in the measuring of standing on the right lower limb there were found tendencies to decrease the median average of COP deflection speeds in the mediolateral direction. From the point of view of the subjective perception of dizziness and the quality of life of patients with dizziness there was found a significant improvement by using the DHI questionnaire.
