Borderline personality disorder, its treatment, and role of the therapeutic relationship

Abstract:

Hraniční porucha osobnosti (HPO) je kontroverzní diagnóza, která byla důkladně studována po celá desetiletí. I přes pozornost věnovanou této poruše, stále existuje mnoho nezodpovězených otázek o její etiologii a léčbě. Odborníci se věnují vývoji speciálních léčebných modelů, které by vyhovovaly potřebám této populace a tak dosáhli pozitivnějších výsledků léčby. Nicméně, výzkum poskytuje rozporné výsledky. Předpokládá se, že žádný psychoterapeutický směr není v léčbě HPO účinnější než jiný. Avšak nedávné výsledky svědčí o tom, že dialektická behaviorální terapie, terapie zaměřená na schéma, psychoterapie zaměřená na přenos a mentalization-based treatment jsou účinné při léčbě této diagnózy. Přesto, není to jen struktura daného přístupu, která má vliv na výsledky terapie. Praktici a vědci navrhují, že terapeutický vztah významně ovlivňuje výsledky terapie. V této teoretické práci se snažíme prezentovat doporučení pro rozvoj a udržení pozitivního terapeutického vztahu s klienty s HPO, který se zdá být klíčovým prvkem jakéhokoli psychoterapeutického přístupu a významně ovlivňuje pozitivní výsledky terapie u této skupiny pacientů.

Abstract:

Borderline personality disorder (BPD) is a controversial diagnosis, which has been thoroughly studied for decades. Despite the attention devoted to this disorder, there are still many unanswered questions about its etiology and its treatment. Practitioners have been working on developing special treatment models that would meet the needs of this population and reach more positive outcomes of the therapy. However, research provides contradictory evidence. It is suggested that no particular psychotherapy orientation is more effective than another in treating BPD. At the same time, recent results provide evidence that dialectical behaviour therapy, schema focused therapy, transference focused psychotherapy, and mentalization based treatment are effective in treating this diagnosis. Nevertheless, it is not only the treatment structure that affects the outcomes. Practitioners and researchers propose that it is the therapeutic relationship that is an important contributor to the therapy outcome. In this theoretical thesis we aim to present an evidence-based guideline for developing and maintaining a positive therapeutic relationship with clients with BPD, which seems to be a crucial element of any therapy approach and significantly influences positive therapy outcomes with this population.
