The influence of IABP on the renal function during septic shock

Abstract:

Akutní renální poškození signifikantně zhoršuje morbiditu a mortalitu pacientů hospitalizovaných na intenzivní péči a její nejčastější příšinou je septický šok. Cílem práce bylo stanovit, jak ovlivňuje použití IABP u nemocných v septickém šoku, který je doprovázený ALCOS (sy nízkého srdečního výdeje) a kde selhávají veškerá léčebná opatření doporučená Surviving Sepsis Campaign, ledvinné funkce. Do souboru bylo zařazeno 10 pacientů, kteří splnili vstupní kritéria septického šoku a ALCOS, po zavedení IABP byly sledovány oběhové parametry: MAP, CI, SvO2, SVR, PVR, VO2I, DO2I, laboratorní parametry: laktát a trombocyty, kreatinin a urea v séru, clearance kreatininu, clearance osmolární a clearance bezsolutové vody, frakční exkrece sodíku, draslíku a vody a klinický parametr: hodinová diuréza, a to 6x v 12 hod intervalech. Výsledky byly statisticky zpracovány testy regresní analýzy na hladině významnosti p < 0,05.

Abstract:

An acute kidney injury significantly adds to morbidity and mortality of patients in the intensive care unit and its most common contributing factor is a septic shock. The aim of this publication is the assessment of the IABP influence on renal function of patients in septic shock that is accompanied by ALCOS (acute low cardiac output syndrome) during Surviving Sepsis Campaign treatment failure. Ten patients who fulfilled the criteria of a septic shock as well as those of ALCOS standards were being observed. After the IABP initiation, the circulatory parameters: MAP, CI, SvO2, SVR, PVR, VO2I, DO2I, the laboratory parameters: lactate and blood platelets, serum creatinine and serum urea, creatinine clearance, osmolal clearance, free water clearance, fractional excretion of sodium, potassium, water, and the clinical parameters: urine output, were being closely observed; 6x in 12-hour intervals. The results were statistically calculated by a regression analysis test at the significance level of p < 0,05.
