Coping strategies, resilience and self esteem of patiens with inflammatory bowel diseases.

Abstract:

Tato diplomová práce pojednává o strategiích zvládání stresu, resilienci a self-esteem u pacientů s nespecifickými střevními záněty, konkrétně Crohnovou chorobou a ulcerózní kolitidou. Teoretická část práce se zaměřuje na vymezení problematiky nespecifických střevních zánětů a na přiblížení teoretických východisek jednotlivých osobnostních charakteristik, přičemž poukazuje na jejich určité souvislosti. Cílem empirické části je pak zmapovat užívané copingové strategie u skupiny respondentů, zjistit míru jejich resilience a self-esteem, a potažmo nalézt mezi těmito proměnnými souvislosti. Dalším cílem je stanovit, zda se tyto fenomény liší v závislosti na pohlaví či typu onemocnění, a v neposlední řadě rozpoznat, jak délka života s diagnózou s nimi souvisí. Na základě stanovených cílů bylo realizováno výzkumné šetření za využití inventáře Strategie zvládání stresu (SVF-78), Stručné škály psychické odolnosti (BRS) a Rosenbergovy škály sebehodnocení (RSES). Výzkumný soubor tvořilo 104 pacientů s nespecifickými střevními záněty různých věkových kategorií i s rozdílnou délkou onemocnění. Výsledky výzkumu ukázaly, že negativní copingové strategie, potřeba sociální opory a strategie vyhýbání se negativně korelují s resiliencí. Dále byl zaznamenán statisticky signifikantní vztah mezi pozitivními copingovými strategiemi, obzvláště strategiemi přehodnocení a devalvace a strategiemi kontroly, a resiliencí. Žádná ze škál pozitivních strategií, kromě škály podhodnocení a pozitivní sebeinstrukce, neprokázala statisticky významné souvislosti se self-esteem. Korelační analýzou však byla identifikována souvislost mezi negativními copingovými strategiemi a self-esteem, a také mezi strategií vyhýbání se a self-esteem. Odhalili jsme také pozitivní korelaci mezi resiliencí a self-esteem. Lineární regresní analýzou jsme identifikovali psychickou odolnost jako významný prediktor pozitivních i negativních copingových strategií, strategie vyhýbání se a potřeby sociální opory. I self-esteem se ukázal jako významný prediktor užívání negativních strategií zvládání stresu, strategie vyhýbání se, a také jako prediktor resilience. Na základě porovnávání fenoménů u mužů a žen bylo zjištěno, že ženy užívají více negativní copingové strategie, kdežto muži disponují vyšší psychickou odolností. Mezi pacienty s Crohnovou chorobou a ulcerózní kolitidou jsme nezaznamenali žádné statisticky významné rozdíly, až na strategii podhodnocení, kterou užívají četněji pacienti s ulcerózní kolitidou. V neposlední řadě se ukázalo, že pacienti s déletrvající diagnózou užívají ke zvládnutí stresu méně strategie kontroly, pozitivní sebeinstrukce a pociťují menší potřebu vyhledávat sociální oporu oproti pacientům žijících s onemocněním kratší dobu.

Abstract:

This master thesis is concerned with coping strategies, resilience and self-esteem among patients with inflammatory bowel disease, specifically Crohn's disease and ulcerative colitis. The theoretical part of the thesis focuses on defining inflammatory bowel disease and on presenting theoretical basis of each personality characteristic, while pointing out on certain connections between them. The aim of the research is to map the coping strategies used by a group of respondents, to determine the level of their resilience and self-esteem, and consequently to find correlations between these variables. A further aim was to find out whether these phenomena differ according to the gender or type of disease, and last but not least to discover the connection between the individual phenomena and the life expectancy with the diagnosis. Based on the stated aims was conducted a survey using the Stress Coping Style Questionnaire (SVF-78), the Brief Resilience Scale (BRS) and the Rosenberg's Self Esteem Scale (RSES). The research sample consisted of 104 patients suffering from inflammatory bowel disease, of different ages and with different types of disease. The results showed that negative coping strategies, need for social support and avoidance strategies are negatively correlated with resilience. Furthermore, there was a statistically significant relationship between positive coping strategies, especially reappraisal and devaluation strategies and control strategies, and resilience. None of the positive coping strategies scales, except the undervaluation and positive self-instruction scales, showed statistically significant relationships with self-esteem. Correlational analyses identified an association between negative coping strategies and self-esteem, and between avoidance strategies and self-esteem. We also revealed a positive correlation between resilience and self-esteem. Linear regression analysis identified psychological resilience as a significant predictor of positive and negative coping strategies, avoidance strategies, and need for social support. Self-esteem also emerged as a significant predictor of the use of negative coping strategies, avoidance strategies, and resilience. By comparing phenomena for men and women, it was found that women use more negative coping strategies, whereas men possess higher psychological resilience. No statistically significant differences were observed between patients with Crohn's disease and patients with ulcerative colitis, except for the underestimation strategy, which is used more frequently by ulcerative colitis patients. Finally, patients with a longer-term diagnosis of IBD appeared to use fewer control strategies, positive self-instruction, and felt less need to seek social support to cope stress compared to patients living with the disease for a shorter period of time.
