Hierarchy of attachment figures among drug users

Abstract:

Práce si klade za cíl popsat specifika citové vazby k matce a specifickou hierarchii citové vazby u uživatelů návykových látek se syndromem závislosti. Dotazníkového šetření se zúčastnilo 61 klientů psychiatrických nemocnic postupujících léčbu závislosti a dalších 61 respondentů nevykazujících vzorce problémového užívání drog. Všem účastníkům bylo 18 až 30 let. Byly použity dotazníky ECR-RS a WHOTO. Možná interpretace výsledků t-testu je, že soubor se syndromem závislosti je ve vztahu s matkou úzkostnější a vyhýbavější než soubor bez závislosti. Přesto je matka hlavní vazbovou osobou pro osoby se závislostní poruchou. Interpretace výsledků testů nezávislosti χ2 pro čtyřpolní tabulku (kontingenční tabulku 2x2) naznačují, že v porovnání s kontrolním souborem mají uživatelé návykových látek vyšší potřebu udržování blízkosti k matce, ale neliší se v obsazování matky do funkce jisté základny. U kontrolního souboru jsou hlavní vazbovou osobou partneři, je u nich završen přesun funkcí citové vazby od rodičů k vrstevníkům. Byl zjištěn rozdíl mezi soubory v umísťování partnerů a vrstevníku do funkce jisté základny. Uživatelé návykových látek je umísťovali do této funkce méně často než respondenti z kontrolní skupiny. Naopak soubory se nelišily v obsazování vrstevníků do funkce udržování blízkosti. V léčebném prostředí psychiatrické nemocnice se terapeuti nestávají hlavní vazbovou osobou a nejsou dosazováni do funkcí citové vazby, což by mělo být zohledněno při indikaci klientů s poruchou attachmentu do léčebných zařízení. Ze zjištění lze vyvodit závěr, že na syndrom závislosti je relevantní (a snad i nutné) nahlížet optikou teorie attachmentu a neukončených vývojových úkolů, které jsou častou překážkou při úzdravě nemocných.

Abstract:

Thesis aims to describe the specific bonds towards mother and hierarchy of attachment figures among drug users with drug use disorder. Questionnaire has been responded by 61 clients of psychiatric wards during their addiction’s treatment along with another 61 respondents without any patterns of drug abuse. All participants were between 18 to 30 years of age and ECS-RS and WHOTO questionnaires were used. Possible interpretation of t-test shows that respondents with addiction disorder are more anxious and avoidant in relationship with their mother than respondents without addiction. Mother is, however, primary attachment figure for the persons with drug use disorder. Interpretation of the results of the chi-squared independent test for 2x2 contingency table indicate that the drug users tend more to seek proximity to their mother when compared to the control group. Whereas they do not differ in posing mother into secure base. Within the control group the primary attachment figure is partner with finished transfer of attachment functions from parents to peers. A difference between the groups has been found in placing partners and peers into secure base. Drug users had placed them there less often than members of the control group. On the other hand the groups did not differ in placing peers to proximity seeking function. Therapists are neither posed as the primary attachment figure nor into the function of secure base in the therapeutic environment of a psychiatric ward. This should be taken in account when referring clients with attachment disorder to a medical facility. We may conclude from the findings that the drug use disorder should be viewed through the lens of the attachment theory and through uncompleted development tasks which are often an obstacle to cure the sick.
