The rehabilitation of a patient with a simple olecranon elbow fracture and traumatic detachment of the humeral triceps tendon: clinical case

Abstract:

Olecranon fractures are very common in adults and the rupture mostly affects the proximal apex of the ulna. These fractures are caused by a direct trauma or by a fall with the elbow in hyperextension. Olecranon fractures are divided into compound fractures, stable and unstable displacements. Compound fractures are not usually subjected to a surgical operation but they are resumed through conservative treatment, unlike those stable and unstable ones. However, the ruptures of the triceps tendon are very rare and they are treated surgically to not lose the extension of the elbow. The injury to this tendon is caused by a sudden eccentric muscle contraction. The diagnosis of tendon rupture is verified by ultrasound or magnetic resonance imaging. The thesis will deal with the anatomy of the upper limb then the shoulder complex, the elbow complex and finally the wrist and hand. Subsequently, the pathologies related to olecranon fractures will be explained putting in evidence what they entail and how they are treated; the same thing will be done for the rupture of the triceps tendon. In the final analysis, "literature" treatments will be compared with those performed on a patient who reported a fracture of the olecranon to assess its effectiveness for a return to normal musculoskeletal joint activity.

Abstract:

Le fratture di olecrano sono molto comuni negli adulti e la rottura interessa maggiormente l'apice prossimale dell'ulna. Queste fratture sono provocate attraverso un trauma diretto o tramite una caduta con il gomito in iperestensione. Le fratture olecraniche si suddividono in fratture composte, scomposte stabili e scomposte instabili. Le fratture composte solitamente non vengono sottoposte a una operazione chirurgica ma vengono riprese attraverso un trattamento conservativo, a differenza di quelle scomposte stabili e instabili. Le rotture del tendine del tricipite, invece, sono molto rare e sono trattate chirurgicamente in modo tale da non perdere l'estensione di gomito. La lesione di questo tendine ? provocata da una contrazione muscolare eccentrica improvvisa. A verificare la diagnosi della rottura del tendine ? l'ecografia o la risonanza magnetica. La tesi tratter? l'anatomia dell'arto superiore quindi il complesso della spalla, il complesso del gomito e infine polso e mano. Successivamente, verranno esposte le patologie legate alle fratture di olecrano spiegando cosa comportano e come vengono trattate; questo viene svolto anche per la rottura del tendine del tricipite. In ultima analisi, saranno comparati i trattamenti "di letteratura" con quelli effettuati su un paziente che ha riportato una frattura dell'olecrano per valutarne l'effettiva efficacia per un ritorno alla normale attivit? dell'articolazione muscolo-scheletrica
