The importance of airborn allergens in the flares of atopic dermatitis

Abstract:

Je známo, že aerogenní alergeny (aeroalergeny) mohou u části pacientů vyvolat exacerbaci atopického ekzému. K průkazu účasti těchto alergenů se začíná používat nová diagnostická metoda spočívající v přímé expozici kůže atopika suspektním aeroalergenům. Tato metoda se nazývá atopické epikutánní testy (APT). V dermatoalergologické ambulanci I.dermatovenerologické kliniky FN u sv. Anny v Brně jsme pomocí atopických epikutánních testů vyšetřili v období od listopadu 2004 do dubna 2006 celkem 100 pacientů s atopickým ekzémem ve věku od 16 do 70 let (z toho bylo 26 mužů a 74 žen). U 32 (32 %) pacientů byly atopické epikutánní testy pozitivní, u 68 (68 %) negativní. Nejčastěji zjišťovanými alergeny byly roztoči domácího prachu (13 %), na druhém místě pyly bylin a travin (12 %) a na třetím místě psí alergeny (10 %). Korelace výsledků atopických testů s anamnestickými údaji pacientů ve smyslu zhoršování ekzému aerogenními vlivy byla statisticky signifikantně vyšší u atopických epikutánních testů (66 %) než u vyšetření specifického IgE (42 %). U podskupiny pacientů, kde byly provedeny i prick testy (n=23), výsledky atopických epikutánních testů korelovaly s anamnestickými údaji pacientů v 69,5 %, zatímco korelace výsledků prick testů a specifického IgE na aerogenní alergeny s anamnézou pacientů byla nižší (60,9 % resp.56,5 %). Atopické epikutánní testy mají vyšší specifitu oproti prick testům a specifickému IgE a vhodným způsobem doplňují a zpřesňují možnosti identifikace aeroalergenů. Na základě nášich zjištění se tedy identifikace aeroalergenů jako možných zhoršujících faktorů atopického ekzému pomocí atopických epikutánních testů jeví být velmi přínosnou.

Abstract:

It is known that airborn allergens may elicit the flare of atopic dermatitis (AD). A new diagnostic method consisting in a direct exposition of the atopic skin to the airborn allergens has been recently introduced to the clinical practice. This method is called the atopy patch tests (APT). Between November 2004 and April 2006 a group of 100 patients (26 males and 74 females) with atopic dermatitis aged from 16 to 70 years was investigated with atopy patch tests in the dermato-allergological office of the Ist. dept. of Dermatovenereology of the St. Anna Faculty Hospital in Brno. In 32 (32 %) patiens the APT were positive, in 68 (68 %) negative. The most common allergens were the house dust mite allergens (13 %), the second were grass and plant pollen (12 %) and the dog allergens were on the third place (10 %). The correlation of the results of APT with the history of flares after the contact with the airborn allergens was statistically significantly higher in APT (66 %) than in specific IgE (42 %). In a subgroup of patiens where also prick tests were carried out (n=23), the results of atopy patch tests correlated with the history of patiens in 69,5 % , while the correlation of the results of prick tests and specific IgE to airborn allergens with the history was lower (60,9 % resp.56,5 %). Atopy patch tests have higher specifity in comparison with prick tests and specific IgE and they are therefore able to supplement and precise the possibilities of the identification of aeroallergens. On the basis of the knowledge obtained from this research, the identification of airborn allergens as possible triggering factors of atopic dermatitis with atopy patch tests appears to be very atvantageous.
