Abstract:

Předkládaná dizertační práce se zaměřuje na jazykové schopnosti u lidí se schizofrenním onemocněním. Práce sestává z teoretického úvodu a třech dílčích kvalitativně-kvantitativních empirických studií, přičemž ve všech je použitá některá z psychodiagnostických metod testujících jazykové procesy, následná kategorizace verbálních dat pomocí klasifikačních systémů a porovnání skupiny osob se schizofrenií s osobami z neklinické populace. Použitými psychodiagnostickými metodami jsou Test slovních asociací, Test verbální fluence a subtest Slovník (z Wechslerovy inteligenční škály pro dospělé). Jazykové schopnosti u lidí se schizofrenií vykazují určité odlišnosti od osob bez duševního onemocnění, které se vyskytují napříč všemi jazykovými rovinami a společně s deteriorací funkcí jiných kognitivních domén mohou mít dopad na úspěšnost terapie a kvalitu života jedince (Green et al., 2000; Marini et al., 2008; Niznikiewicz et al., 2013; Schwartz, 2014). Cílem této dizertační práce bylo prozkoumat některé tyto popisované odlišnosti, zejména z roviny lexikálně-sémantické, u osob z české populace. První studie se věnuje tzv. clang odpovědím (čili fonologickým asociacím) v Testu slovních asociací, které se dle závěrů předešlých studií vyskytují častěji právě u lidí se schizofrenií (Covington et al., 2005). Výsledky naší analýzy ukázaly, že ve vzorku osob se schizofrenií bylo sice statisticky významně více odpovědí typu clang v porovnání s lidmi z neklinické populace i osobami s jiným typem psychotického onemocnění, nicméně vždy šlo o výskyt v řádu jednotek a rozpětí hodnot bylo velmi malé, proto nelze tyto odpovědi považovat za spolehlivý, jednoznačně rozlišující rys. Druhá studie se věnovala analýze odpovědí v Testu verbální fluence z pohledu systematičnosti vybavování, tedy tzv. clusterování a switchování (Troyer et al., 1997; da Silva et al., 2004). Porovnávali jsme způsob odpovídání mezi osobami se schizofrenií a bez duševního onemocnění, ale také ve verbální a neverbální verzi této úlohy. Výsledky v souladu s předešlými výzkumy potvrdily slabší výkon u lidí se schizofrenií z pohledu počtu odpovědí, clusterů i switchů ve všech verzích fluence, nicméně nejnápadnější odlišnost byla zachycena v kategoriální verbální fluenci, kde osoby se schizofrenií měly významně méně procent slov zařazených v clusterech a navíc jejich strategie byla stejná jako u dalších dvou verzí fluence (fonematické a neverbální), zatímco u lidí z neklinické populace byla naznačena tendence uplatňovat při odpovídání v kategoriální fluenci odlišnou strategii. Třetí studie stavěla na odpovědích ze subtestu Slovník, které byly analyzovány z pohledu Významového systému manželů Kreitlerových (1990a), konkrétně významových dimenzí. V souladu s výsledky studie těchto izraelských autorů (srov. Kreitler & Kreitler, 1986) jsme zaznamenali u lidí se schizofrenií obecně tendenci dávat méně obsažné definice, nicméně se od neklinické populace nelišily v kvalitě použitých významových dimenzí či jejich pořadí. Výsledky všech námi provedených studií naznačují u lidí se schizofrenií určitý specifický trend ve stylu odpovídání v použitých psychodiagnostických metodách, nicméně jednoznačná, zcela průkazná odlišnost nebyla potvrzena. V porovnání s lidmi z neklinické populace byla v našich studiích u lidí se schizofrenií nejnápadnější celková stručnost jazykového projevu, která může souviset s ochuzelostí slovní zásoby, jež bývá popisována v teoretických statích.

Abstract:

This thesis focuses on the language abilities in people with schizophrenia. It consists of theoretical introduction and three qualitatively-quantitative empirical studies. Each of them uses certain psychodiagnostic method testing language functions, subsequent categorization of verbal data by classifications and final comparison of people suffering from schizophrenia with nonclinical population. Three psychodiagnostic instruments are used: Word association test, Verbal fluency test and Vocabulary subtest (included in Wechsler Adult Intelligence Scale). Some disturbances in language affecting all language levels in people with schizophrenia compared to people without mental illness are reported, and together with deterioration in other cognitive domains, these may have an impact on patients’ treatment success and quality of life (Green et al., 2000; Marini et al., 2008; Niznikiewicz et al., 2013; Schwartz, 2014). The aim of this thesis was to study some of these disturbances, mainly the lexical-semantic level, in Czech population. First study focuses on the clang responses (phonological associations) in Word association test. Some of the previous studies showed that clang responses have occurred more frequent among people with schizophrenia (Covington et al., 2005). Our results showed significantly higher incidence of clang responses in people with schizophrenia compared to nonclinical population and also in comparison with people with other psychotic illness. However, clang responses occurred in the order of units and the range of the data was small, so these answers cannot be considered a reliable unequivocal distinguishing feature of schizophrenia illness. Second study focuses on analysis of responses produced in the Verbal fluency test, specifically, clustering and switching (Troyer et al., 1997; da Silva et al., 2004). The specific way of answering of people with schizophrenia and people without mental illness was compared together with the difference between the verbal and nonverbal task. Our results were in accordance with the previous studies and showed poorer performance among people with schizophrenia. In all versions of fluency task, the clinical population produced fewer responses, clusters and switches, however, the most salient difference was in semantic verbal fluency task, where people with schizophrenia produced fewer responses in clusters, moreover their strategy did not differ from the strategy in phonemic and nonverbal task, while the people from nonclinical population showed the tendency to change the strategy in semantic fluency task. Third study is based on the responses from Vocabulary subtest. The responses were analysed using Kreitlers’s Meaning system (1990a), specifically meaning dimensions. Out results were in accordance with their study (see Kreitler & Kreitler, 1986) and showed that the people with schizophrenia produced fewer comprehensive definitions, however, they did not differ in quality and order of meaning dimensions from nonclinical population. Overall results of this thesis indicate a specific trend in the way people with schizophrenia perform in used psychodiagnostic methods, however, a clear and unambiguous difference was not found. The most salient difference between the schizophrenia patients and nonclinical population was found in the speech terseness, which may be related to smaller vocabulary of schizophrenia patients often mentioned in literature.
