Biomarkers of neuropathic pain

Abstract:

Tato práce je primárně zaměřena na hledání biomarkerů neuropatické bolesti a rizikových faktorů jejího rozvoje vč. jejích komorbidit a to především u pacientů s diabetickou polyneuropatií. Práce shrnuje aktuální poznání o relativně nových metodách užívaných v diagnostice neuropatie tenkých nervových vláken a neuropatické bolesti, jako jsou korneální konfokální mikroskopie, kvantitativní testování senzitivity a nociceptivní evokované potenciály. Klíčové výstupy publikovaných studií komentovaných v rámci této práce jsou následující: Přítomnost neuropatické bolesti u diabetické polyneuropatie je asociována především s vyšší tíží polyneuropatie, s výskytem deprese a úzkosti, s vyšším věkem pacientů, s ženským pohlavím a laboratorními známkami nefropatie. Neuropatická bolest představuje nejvýznamnější rizikový faktor rozvoje deprese a úzkosti u diabetické polyneuropatie. Dalšími rizikovými faktory pro rozvoj deprese a úzkosti u pacientů s diabetickou polyneuropatií jsou ženské pohlaví, nižší věk pacientů, přítomnost diabetu druhého typu a vyšší tíže polyneuropatie. Důležitými biomarkery neuropatické bolesti jsou dle recentních poznatků senzitivní fenotypy získané z analýzy výsledků kvantitativního testování senzitivity. U diabetické polyneuropatie převažuje tzv. deaferentační fenotyp a jemu podobný fenotyp se ztrátou percepce senzitivních modalit vycházející z novější klasifikace. Oba se vyskytují především u bolestivé formy diabetické polyneuropatie a jsou také spojeny s klinicky závažnějšími formami diabetické polyneuropatie.

Abstract:

This thesis is focused on searching for biomarkers of neuropathic pain (predominantly related to diabetic neuropathy), and the risk factors for its development and comorbidities. It summarizes the current knowledge of relatively new diagnostic methods of small fibre neuropathy and neuropathic pain, such as corneal confocal microscopy, quantitative sensory testing, and nociceptive evoked potentials. Key findings of the commented published papers are the following: Presence of neuropathic pain in diabetic peripheral neuropathy is mainly associated with the higher severity of neuropathy, depression and anxiety, higher age of patients, female sex, and laboratory signs of nephropathy. Even though neuropathic pain is the main contributor to emotional distress in diabetic neuropathy, other risk factors for development of depression and anxiety independent of pain were identified and include female sex, lower age of patients, type 2 diabetes mellitus, and higher severity of diabetic neuropathy. Important biomarkers of neuropathic pain are sensory phenotypes derived from findings of quantitative sensory testing. The phenotype of deafferentation and a very similar sensory profile, called sensory loss phenotype, prevail in diabetic sensorimotor peripheral neuropathy, mainly in its painful form, and they are connected to more severe forms of diabetic neuropathy.
