Abstract:

Tématem diplomové práce je dostupnost zdravotně sociálních služeb v Jihomoravském kraji pro osoby se schizofrenií. Analyzovat dostupnost zdravotních a sociálních služeb u osob se schizofrenií z pohledu zaměstnanců a uživatelů služeb na Brněnsku a Jižní Moravě. Analyzovat spolupráci mezi organizacemi a zdravotními pracovníky. Teoretická část popisuje základní pojmy týmové spolupráce, možnosti a dopady v praxi, vymezuje základní pojmy související s tématem, jako jsou duševní zdraví a onemocnění, oblast psychiatrie, koordinovaná rehabilitace. Dále jsou obsaženy poznatky o spolupráci v mezioborovém týmu pracujícímu s lidmi s duševním onemocněním, o složení týmu v péči či rehabilitaci o duševně nemocné. Empirická část se opírá o poznatky z teoretické části. Cílem empirické části je zjistit, jak jsou zdravotničtí a sociální pracovníci informování o následné možnosti péče po propuštění z nemocnice. Přechod do ambulantní péče, možnosti získávání práce, bydlení, sociálního začlenění. Výzkumné šetření bylo provedeno formou kvantitativní studie, dotazníkové šetření vlastní konstrukce, opírající se o dostupné zdroje.

Abstract:

The topic of the thesis is the availability of health and social services in the South Moravian Region for people with schizophrenia. To analyze the availability of health and social services for people with schizophrenia from the perspective of employees and users of services in the Brno and South Moravia regions. Analyze cooperation between organizations and health professionals. The theoretical part describes the basic concepts of team cooperation, possibilities and impacts in practice, defines the basic concepts related to the topic, such as mental health and disease, psychiatry, coordinated rehabilitation. There is also knowledge about cooperation in interdisciplinary team working with people with mental illness, about composition of team in care or rehabilitation of mentally ill. The empirical part is based on knowledge from the theoretical part. The aim of the empirical part is to find out how health and social workers are informed about the follow-up aftercare from hospital. Transition to outpatient care, possibilities of finding work, housing, social inclusion. The research was carried out in the form of a quantitative study, a questionnaire survey of own construction, based on available resources.
