Analysis of time trends of selected cancer diagnoses in Czech republic

Abstract:

Tato diplomová práce je věnována problematice hodnocení časových trendů, která je nedílnou součástí komplexní analýzy epidemiologických dat. Analýza časových trendů může být využita pro nalezení rizikových či protektivních faktorů daného onemocnění, zhodnocení efektivity diagnostických a léčebných metod nebo pro predikci vývoje onemocnění do budoucna. Předložená diplomová práce přináší ucelený přehled statistických metod využívaných k analýze trendu v epidemiologii, včetně jejich softwarové implementace. V praktické části byly vybrané metody aplikovány na data Národního onkologického registru a byla provedena detailní analýza dvou onkologických diagnóz – zhoubných novotvarů tlustého střeva a konečníku a zhoubných novotvarů prsu u žen. Cílem praktické části bylo zhodnocení vývoje základních epidemiologických ukazatelů v dlouhodobém časovém trendu a posouzení dopadu screeningových programů pro tato onkologická onemocnění. K hodnocení trendu byla v praktické části využita metoda joinpoint regrese, jump modely, Poissonova regrese a age-period-cohort modely. Bylo prokázáno, že pro zhoubné novotvary tlustého střeva a konečníku dochází v posledních letech ke statisticky významnému snižování incidence i mortality. Při využití věkové standardizace dle světového standardu bylo zaznamenáno snižování incidence o 1,5 % ročně (IS: −1,8 %; −1,2 %) a mortality o 3,6 % ročně (IS: −3,8 %; −3,3 %). Pozorovaný pokles byl způsoben především inovací diagnostických a léčebných metod, přičemž screeningový program nebyl hlavním faktorem tohoto poklesu. Pro zhoubné novotvary prsu u žen došlo v posledních letech ke stabilizaci incidence a statisticky významnému snižování mortality o 2,4 % ročně (IS: −2,7 %; −2,2 %). Pozorované změny v trendu byly opět způsobeny zejména pokrokem v léčebných a diagnostických metodách. Hodnocený vliv screeningu se zde ukázal jako výraznější faktor než v případě zhoubných novotvarů tlustého střeva a konečníku. Vzhledem k tomu, že pro zhoubné novotvary tlustého střeva a konečníku i pro zhoubné novotvary prsu u žen pozorujeme stále narůstající podíl novotvarů in situ a časných stádií, lze předpokládat i nadále pozitivní vývoj incidence a mortality pro tato nádorová onemocnění.

Abstract:

This master thesis deals with the issue of time-trend analysis, which is an indispensable part of complex analysis of epidemiological data. Time-trend analysis can be used to identify risk or protective factors of a given disease, to evaluate the effectiveness of diagnostic and therapeutic methods, or to predict future disease progression. This master thesis presents a comprehensive overview of the statistical methods used to analyze time trends in epidemiology, including their software implementation. In the practical part the selected methods were applied to the National Cancer Registry and a detailed analysis of two oncological diagnoses (colorectal cancer and female breast cancer) was performed. The aim of the practical part was to evaluate the observed time trends of basic epidemiologic indicators and assess the impact of screening programs for those oncological diseases. The methods used in the practical part of this thesis were the joinpoint regression, jump models, Poisson regression and age-period-cohort models. The results showed a statistically significant reduction in incidence and mortality for the colorectal cancer in recent years. Deploying age-standardized rate (world standard population) the decrease by 1.5% per year (CI: −1.8%, −1.2%) in incidence and 3.6% per year (CI: −3.8%, −3.3%) in mortality was observed. The most important factor for the observed decrease were innovations of diagnostic and therapeutic methods. The screening program did not play the major role in the decline. For female breast cancer the stabilization of incidence rates and a statistically significant reduction in mortality rates by 2.4% per year (CI: −2.7%, −2.2%) have occurred in recent years. The observed changes in female breast cancer were again due to the progress in treatment and diagnostic methods. The effect of screening proved to be a more significant factor than in the case of colorectal cancer. Due to the increasing detection of carcinoma in situ and early stages of neoplasms in both colorectal cancer and female breast cancer it can be assumed that the positive progression of those oncological diseases will continue.
