Condraindication of system trombolysis of patients with stroke (Monitoring reasons why wasn´t givn system trombolysis)

Abstract:

V diplomové práci se zaměřuji na sledování pacientů s ischemickou cévní mozkovou příhodou (iCMP) a sledováním důvodů, proč u nich nebyla podána systémová trombolýza. Práce je rozdělena na část teoretickou a výzkumnou. V teoretické části je popsána definice, epidemiologie, fyziologie mozkového oběhu a dělení cévních mozkových příhod (CMP), kde nadále podrobně rozepisuji ischemickou CMP. O hemoragické CMP se zmiňuji velmi stručně, neboť u těchto cévních mozkových příhod nemůže být podána systémová trombolýza. Pokračuji rizikovými faktory iCMP, projevy a příčinami mozkové ischemie, klinickým obrazem, diagnostikou, terapií a prevenci mozkových ischemických iktů. Ve výzkumné části sleduji dané kontraindikace u žen a u mužů. U žen to jsou: věk, čas, dekompenzovaná hypertenze, NIHSS skóre, antikoagulační terapie, stav po CMP, nízké trombocyty a tumor. U mužů jsem sledovala tyto kontraindikace: věk, čas, dekompenzovaná hypertenze, NIHSS skle, antikoagulační terapie, stav po CMP, nízké trombocyty, tumor, INR nad 1,7, úraz a aneurysma. V této části jsem si stanovila 4 výzkumné otázky, 4 pracovní hypotézy a 2 hypotézy. Zaměřuji se také na daný věk pacientů. Dále zkoumám, kolik pacientů z celkového počtu trombolýzu absolvovalo.

Abstract:

ANNOTATION The diploma thesis is focused on monitoring patients with ischemic cerebrovascular accident (iCVA) and the reasons why they were not given systemic thrombolysis. The thesis is divided into theoretical and research part. The theoretical part of the thesis provides the definition of brain circulation, its epidemiology and physiology, as well as the division of cerebrovascular accidents (CVA), paying close attention to ischemic CVA. Hemorrhagic CVA is mentioned briefly since systemic thrombolysis cannot be given with this kind of cerebrovascular accidents. The theoretical part then continues with risk factors of iCVA, symptoms and causes of cerebrovascular ischemy, clinical picture, diagnosis, therapy and prevention of cerebral ischemic stroke. The research part monitors the given contraindications in men and women. Regarding women, these are: age, time, decompensated hypertension, National Institutes of Health Stroke Scale (NIHSS) score, an anticoagulant treatment, a condition after CMP, low thrombocytes and tumour. Regarding men, the following contraindications were monitored: age, time, decompensated hypertension, National Institutes of Health Stroke Scale (NIHSS) score, an anticoagulant treatment, a condition after CMP, low thrombocytes, tumour, INR above 1,7, injury and aneurysm. There are 4 research question, 4 pre-hypotheses and 2 hypotheses stated in this part. The age of the patients is in focus as well. What follows is monitoring the number of patients taking the thrombolysis.
