Operační terapie chronické skafolunátní disociace modifikovanou dorzální ligamentoplastikou

Chronic scapholunate dissociation: treatment with modified dorsal ligamentoplasty

Abstract:

Cíl: Zhodnocení výsledků operačního řešení chronické skafolunátní disociace (SLD) modifikovanou dorzální ligamentoplastikou pomocí prospektivní studie. Materiál a metodika: Čtyřicet pacientů s chronickou SLD (23 pacientů s dynamickou formou a 17 pacientů statickou formou), u kterých byla provedena náhrada skafolunátního vazu šlachovým štěpem technikou modifikované dorzální ligamentoplastiky, se dostavilo k závěrečnému pooperačnímu hodnocení. V souboru bylo 29 mužů a 11 žen s průměrným věkem 43 let. Pacienti byli hodnoceni předoperačně a poté pooperačně s minimálním odstupem 12 měsíců od výkonu. Bolest dle VAS, rozsah pohybu v zápěstí, síla stisku, DASH skóre a Martini skóre (skórovací systém pro karpální nestability) byli parametry použity k zhodnocení efektu terapie. Výsledky: Pooperačně byl v obou podsouborech (podsoubor statické a dynamické formy) zaznamenán ústup bolesti dle VAS, zvětšení síly stisku a pokles hodnoty DASH. Zlepšení všech parametrů bylo statisticky významné v porovnání s předoperačním stavem. Změny v rozsahu pohybu zápěstí nebyli statisticky významné. Pooperační hodnota Martini skóre dosáhla 27 bodů v podsouboru dynamické formy a 25 bodů v podsouboru statické formy. Nebyl pozorován statisticky významný rozdíl v sledovaných pooperačních parametrech mezi podsouborem dynamické a statické formy chronické SLD. Závěr: Modifikovaná dorzální ligamentoplastika je operační technika, která dostatečně obnovuje stabilitu skafolunátního skloubení a zlepšuje celkový funkční stav zápěstí u pacientů s dynamickou i statickou formou SLD. Výsledky jsou zcela porovnatelné s jinými operačními technikami používanými k terapii chronické SLD.

Abstract:

Purpose: To present the clinical results of a prospective study of chronic scapholunate dissociation treated with modified dorsal ligamentoplasty (tendon graft replacement of scapholunate ligament). Material and Methods: Forty patients with chronic scapholunate dissociation (23 with diagnosis of dynamic form and 17 with static form) treated with modified dorsal ligamentoplasty were available for follow-up evaluation. The ratio of men to women was 29:11. The mean age at the time of operation was 43 years. Patients were examined preoperatively and then postoperatively with minimal follow-up of 12 months. Pain on VAS, grip strength, range of wrist motion, DASH score and Martini score (carpal instability scoring system) were assessed parameters. Results: Postoperative pain relief, increase of postoperative grip strength, improvement of DASH score were statistically significant when compared with preoperative values in both (dynamic and static form) groups. Changes in range of wrist motion were not statistically significant. Postoperative Martini score averaged 27 points in dynamic form group and 25 in static form group. No statistically significant differences in postoperative pain, range of wrist motion, grip strength, DASH or Martini scores between static and dynamic form groups were noted. Conclusions: Modified dorsal ligamentoplasty provides sufficient restoration of stability, pain relief and wrist functional improvement for patients with both forms (dynamic and static) of chronic scapholunate instability. The results are comparable with the outcomes of other clinical reviews presenting scapholunate instability treatment with other different surgical techniques.
