Angiogenesis in tumors: evaluation of predictive factors in patiens with colorectal cancer

Abstract:

Východiska. Mutace genu KRAS je důležitým faktorem pro biologii a léčbu metastatického kolorektálního karcinomu (mCRC). Úloha genu KRAS v léčbě bevacizumabem v kombinaci s chemoterapií není dosud dobře známa. Cílem studie „A“ bylo posoudit klinický výsledek a rizikové faktory ovlivňující výsledky léčby v závislosti na stavu genu KRAS. T-regulační lymfocyty (T-reg) díky svým imunosupresivním vlastnostem mají velký význam při úniku nádorových buněk mechanismům imunitní kontroly. Cytotoxické CD8+ lymfocyty jsou klíčovými efektory protinádorové imunity. Ve studii „B“ uvádíme zjištěné hladiny T-reg u referenční populace a u pacientů s mCRC a zkoumáme, zda hladina T-reg a/nebo cytotoxických lymfocytů může predikovat léčebné výsledky první linie léčby mCRC. Metody. Analýza studie „A“ byla provedena za použití dat 1622 pacientů s mCRC léčených bevacizumabem a chemoterapií na bázi oxaliplatiny či irinotekanu. Výsledky léčby byly hodnoceny v závislosti na mutačním stavu genu KRAS. Ve studii „B“ byl analyzován počet CD8+, T-reg a jejich poměr v oběhu ve vztahu k predikci efektivity terapie u pacientů s mCRC a referenční skupiny. Výsledky. Mutace genu KRAS byla přítomna u 40,6 % pacientů s mCRC. Medián PFS u pacientů s wild-type KRAS (wt KRAS) byl 11,5 měsíců vs. 11,4 měsíce u pacientů s mutací genu KRAS (mut KRAS). Medián OS byl 30,7 vs. 28,4 měsíců (p = 0,312). Pacienti s mutací genu KRAS měli plicní metastázy častěji než jedinci s wt KRAS (32,0 % vs. 23,8 %, p = 0,001). Kombinace chemoterapie na bázi oxaliplatiny a bevacizumabu vedla k hraničnímu zvýšení OS ve srovnání s kombinací bevacizumabu a chemoterapie na bázi irinotekanu (HR = 0,81, p = 0,052). Zastoupení ani absolutní počet T-reg a cytotoxických T-lymfocytů v periferní krvi se v průběhu první linie léčby mCRC neměnil. Zastoupení T-reg, cytotoxických CD8+ Tlymfocytů, jejich absolutní počet a poměr CD8/T-reg nebyl spojen s efektem na PFS či OS. Při léčbě bevacizumabem a chemoterapií byl medián přežití bez progrese u podskupiny pacientů s nízkým počtem T-regulačních lymfocytů 34,6 měsíců oproti PFS 19,9 měsíců u pacientů s vysokou hladinou T-regulačních lymfocytů v periferní krvi (p = 0,1811). Medián celkového přežití byl u podskupiny pacientů s nízkým počtem T-regulačních lymfocytů 69,1 měsíců oproti 34 měsícům u pacientů s vysokou hladinou T-regulačních lymfocytů v periferní krvi (p = 0,0476). Závěr. U pacientů s metastatickým kolorektálním karcinomem nebyl klinický přínos první linie léčby bevacizumabem a chemoterapií ovlivněn mutací genu KRAS a pacienti s nízkým počtem T-regualčních lymfocytů v periferní krvi měli delší celkové přežití.

Abstract:

Background. Activating mutation of the KRAS gene is an important factor in CRC biology and treatment. The role of KRAS status in the treatment with bevacizumab plus chemotherapy is yet not well established. The aim of the present retrospective study was to analyze clinical outcome and risk factors associated with treatment outcomes according to KRAS status. T-regulatory lymphocytes are important players in escape of tumor cells from immune surveillance. Cytotoxic CD8+ lymphocytes are key efectors in anticancer immunity. We describe T-reg level in reference population and in colorectal cancer patients and we investigated whether T-reg and/or cytotoxic lymphocyte level predict outcome of first line treatment of metastatic colorectal cancer. Methods. We analyzed data from 1622 patients with mCRC treated with bevacizumab plus oxaliplatin- or irinotecan-based chemotherapy, and correlated treatment outcomes with KRAS mutation status. We describe T-reg level in reference population and in colorectal cancer patients and we investigated whether T-reg and/or cytotoxic lymphocyte level predict outcome of first line treatment of mCRC. Results. Mutation in KRAS was present in 40.6 % of mCRC cases. The median PFS in patients with wild-type KRAS (wt KRAS) vs mutant KRAS was 11.5 vs 11.4 months, respectively. The median OS was 30.7 vs 28.4 months (p = 0.312). Patients with KRAS mutation had lung metastases more frequently than wt KRAS individuals (32.0 % vs 23.8 %; p = 0.001). Combination of oxaliplatin-based chemotherapy with bevacizumab resulted in a borderline increase in OS over bevacizumab/irinotecan backbone (HR = 0.81, p = 0.052) caused by worse OS in mutant KRAS patients treated with bevacizumab/irinotecan-based chemotherapy. Proportions and absolute counts of T-regs and cytotoxic T-cells in the peripheral blood were not influenced by the first-line treatment of mCRC. Count of T-reg cells, CD8+ T cells, their absolute number and CD8+/T-reg ratio was not associated with an effect on PFS and OS. Treatment with bevacizumab and chemotherapy resulted with median PFS 34.6 months in the subgroup of patients with low count of T-regs as compared to 19.9 months in patients with high levels of T-regs (p = 0.1811). Median OS in the subgroup of patients with low T-reg count was 69.1 months versus 34 months for patients with high levels of T-regs in the peripheral blood (p = 0.0476). Conclusion. In metastatic colorectal cancer patiens treated in the first line with bevacizumab and chemotherapy, KRAS mutation does not interfere with clinical benefit and low absolute T-regulatory lymfocyte count in peripheral blood is aasociated with better oveall survival.
