The health risks of legionella from industrial technologies

Abstract:

Dizertační práce zjišťuje, zda u profesně exponovaných osob v ČR existuje riziko onemocnění legionelózou, jak je toto riziko vysoké a kterých profesí se týká. Hodnotí legislativní, technické a technologické požadavky pro provoz rizikových zařízení. Navrhuje preventivní postupy a doporučení k zajištění bezpečného pracovního prostředí z hlediska výskytu biologických činitelů, včetně legionel. Cílem práce bylo posoudit dostupné nálezy bakterií rodu Legionella ze vzorků technologických vod a jejich klinický význam pro exponované osoby v pracovním prostředí. Byly vybrány a dále analyzovány protokoly vydané Národní referenční laboratoří pro legionely. Ve všech zkoumaných technologiích (chladicí věže, výroba plastů vstřikováním a výroba plochého skla) byly legionely zjištěny, a to i kmeny patogenní pro člověka včetně Legionella pneumophilla sg. 1, Pontiac. K posouzení zdravotního významu těchto nálezů byla analyzována statistická data o nemocnosti legionelózou v obecné populaci ČR a o legionelóze jako uznané a ohlášené nemoci z povolání. Dále byla provedena rešerše profesionálních onemocnění legionelózou z odborné literatury. Bylo zjištěno, že onemocnění osob po profesionální expozici jsou sporadicky zjišťována. Za rizikové lze považovat minimálně zaměstnání: pracovníci ve strojírenské výrobě, ve stavebnictví, řemeslníci, zahradníci a pěstitelé, řidiči z povolání, specialisté a technici v oblasti vědy a techniky, pracovníci ve službách a prodeji. Vhodným preventivním přístupem je zavedení metody hodnocení a řízení biologických rizik do praxe průmyslových provozů.

Abstract:

The dissertation examines the risk of legionellosis in the working environment. It investigates what occupations are concerned and evaluates legislative, technical and technological requirements for the operation of risk facilities. Preventive measures and recommendations ensuring a safe working environment in terms of the biological agents, including Legionella, are suggested. The objective of the thesis was to assess the available findings of Legionella bacteria from the samples of process water and their clinical relevance for the individuals exposed within the working environment. Protocols issued by the National Reference Laboratory were chosen and further analyzed. The presence of Legionella including strains pathogenic for human as Legionella pneumophila sg. 1 was detected in all of the examined facilities (cooling towers, plastics injection molding and production of sheet glass). In order to assess the health significance of these findings statistical data of legionellosis morbidity in the general population and data of legionellosis reported and recognized as a ocupational disease were analyzed. Then, the literature search of occupational legionellosis was carried out. It was found that diseases of occupational exposed individuals are rarely detected. The following occupations can be considered as risk: workers in mechanical engineering or in construction, craftsmen, gardeners and growers, professional drivers, technicians and specialists in the field of science and technology, workers in services. The implementation of methods of evaluation and management of biological risks is an appropriate preventive approach.
