Stereotactic radiotherapy of prostate cancer

Abstract:

Má bakalářská práce je zaměřena na stereotaktickou radioterapii karcinomu prostaty. Jedná se o neinvazivní léčbu pomocí přístroje CyberKnife a je určena pacientům, kterým v minulosti nebylo možné jinak pomoci. CyberKnife je schopen pomocí robotického ramene vysílat svazek velmi tenkých paprsků z téměř každé pozice. Výhodou této léčby je vysoká přesnost s maximálním šetřením okolních zdravých tkání. V první kapitole teoretické části se snažím popsat stereotaktickou radioterapii, kde se zabývám plánováním, srovnáním konvenční radioterapie s radioterapií stereotaktickou a přístrojovým zastoupením. V další kapitole se zaměřuji na limity ve stereotaktické radioterapii, dále kritické orgány, které jsou rozhodující pro oblast prostaty. Ve třetí kapitole je popsána anatomie a funkce prostaty, která postupně přechází k popisu nádorového onemocnění prostaty a k samotné léčbě. Čtvrtá kapitola objasňuje radiosenzitivitu prostaty a popisuje prostatu jako kritický orgán. Poslední kapitola teoretické části je soustředěna na stereotaktickou radioterapii prostaty, kde se podrobně zabývám léčbou a komplikacemi. Cílem mé praktické části je vyhodnocení dotazníku, které byly dány pacientům k vyplnění před léčbou, během léčby a po léčbě.

Abstract:

This thesis is focused on stereotactic radiotherapy of prostate cancer. This is a non-invasive treatment using the CyberKnife device and it is intended for patients who didn´t have chance to be treated by any other methods in the past. CyberKnife is capable to transmit very thin rays from almost any position using a robotic arm. The advantage of this treatment is high accuracy with maximum care to surrounding healthy tissues. In the first chapter of the theoretical part my aim is to describe stereotactic radiotherapy, its planning, comparing conventional radiotherapy with radiotherapy and stereotactic instrument representation. The next chapter focuses on the limits of stereotactic radiotherapy, as well as critical organs that are crucial for the prostate. The third chapter describes the anatomy and function of the prostate. This chapter gradually turns to the description of prostate cancer and the treatment itself. The fourth chapter explains the radiosensitivity of prostate and describes prostate as a critical organ. The last chapter of the theoretical part is focused on stereotactic radiotherapy of the prostate, where I describe treatment and complications in detail. The goal of the practical part is the evaluation of a questionnaire, which were given to patients to fill out before, during and after treatment.
