Imaging methods in diagnostic workup of selected rare oncological diseases

Abstract:

Na základě nařízení Evropského parlamentu a Rady číslo 141/2000 o léčivých přípravcích, jsou jako vzácná onemocnění označovány život ohrožující nebo chronicky invalidizující chorobné stavy s prevalencí nepřesahující 5 postižených osob na 10 000 obyvatel. Tato skupina chorob je velmi heterogenní, často se jedná o dědičná (či vrozená) multisystémová onemocnění. V oboru onkologie do této kategorie spadá několik desítek onemocnění. Některá raritní onkologická onemocnění jsou všeobecně známá (např. akutní leukémie), jiné vzácné diagnózy se vyskytují s prevalencí několika případů na 1 milion obyvatel, či se jejich četnost vyjadřuje jen počtem případů popsaných v odborné literatuře (např. Erdheimova-Chesterova nemoc). Vzhledem k jejich nízké prevalenci v populaci neexistuje o těchto onemocněních dostatek validních vědeckých informací ani neexistují klinické studie zaměřené na jejich terapii. Neboť je povědomí o těchto nemocech mezi odbornou veřejností malé, bývají tyto choroby často diagnostikovány opožděně či chybně. Hlavním cílem předkládané disertační práce je rozšířit poznání o vybraných vzácných onemocněních z pohledu zobrazovacích metod, zejména PET/CT (pozitronová emisní tomografie/výpočetní tomografie), a zvýšit informovanost odborníků. Disertační práce je koncipována jako komentovaný soubor prací, na nichž se uchazečka podílela jako hlavní autor. Každá literární příloha je opatřena krátkým úvodem do problematiky. Hlavním tématem práce je PET/CT diagnostika pěti vzácných onemocnění: mastocytózy, Castlemanovy choroby, epiteloidního hemangioendoteliomu, amyloidózy a Erdheimovy-Chesterovy choroby. Většinou se jedná o popisy případů jednotlivých pacientů, ale součástí je rovněž retrospektivní studie 29 pacientů s Castlemanovou chorobou. Disertační práce se dále zabývá významným přínosem PET/CT při hodnocení léčebné odpovědi. Do této oblasti spadá i část jedné z kapitol, která je věnována nové metodě měření difuzní metabolické aktivity histiocytózy z Langerhansových buněk v plicním parenchymu. Kromě monitorování léčebné odpovědi u pacientů s plicní formou onemocnění má tato metoda také potenciál při screeningu postižení plic u prokázané histiocytózy postihující jiné orgány. Soubor komentovaných prací je rozšířen o dva další články, které se zabývají méně častými přínosy PET/CT vyšetření u pacientů s onkologickou diagnózou, která nespadá do skupiny raritních onemocnění. Součástí práce jsou seznamy dalších odborných výstupů dle jednotlivých témat, na kterých se uchazečka podílela jako spoluautorka. Jsou dokladem několikaletého úsilí v diagnostice vzácných diagnóz a úzké odborné spolupráce mezi radiodiagnostiky a onkology.

Abstract:

According to the Regulation No. 141/2000 of the European Parliament and of the Council on medicinal products, rare diseases are defined as life-threatening or chronically debilitating conditions with prevalence not exceeding five affected persons per 10,000 inhabitants. Often involving hereditary (or inherited) multisystem disorders, this group is very heterogeneous, and in the field of oncology, it includes several dozens of entities. Some rare oncologic diseases are well known (e.g., acute leukaemia), while the prevalence of other reaches to several cases per 1 million inhabitants or is expressed only as a number of cases described in the scientific literature so far (e.g., Erdheim-Chester disease). Given their low prevalence, there is a lack of solid scientific data about these diseases and clinical studies focusing on their treatment. The limited awareness among healthcare professionals often leads to an incorrect or missed diagnosis. Therefore, the main objective of this thesis is to improve knowledge about selected rare diseases from a perspective of imaging methods, especially PET/CT (positron emission tomography/computed tomography) imaging, and to improve professional awareness. The doctoral thesis has been conceptualized as a collection of academic papers with the candidate listed as the first author and includes explanatory commentaries, so that each publication is preceded by a short introduction of the respective topic. The thesis focuses on PET/CT imaging for the initial workup of five rare diseases, comprising mastocytosis, Castleman disease, epithelioid hemangioendothelioma, amyloidosis, and Erdheim-Chester disease. The collection mostly includes case reports but also a retrospective study of 29 patients with Castleman disease. In addition, the thesis aims to document the important role of PET/CT imaging in treatment response evaluation, such as discussed in one of the chapters dedicated to a new method of measuring diffuse metabolic activity of Langerhans cell histiocytosis in the pulmonary parenchyma. Apart from treatment response monitoring, this method can be used as a simple non-invasive screening tool for pulmonary involvement in patients diagnosed with Langerhans cell histiocytosis affecting other organs. The article collection has been complemented by two further publications dealing with a less common role of PET/CT imaging in patients with an oncologic diagnosis not fulfilling the criteria of a rare disease. Categorized according to the topics, the thesis also contains a summary of other research outputs on which the candidate collaborated as a co-author. They testify to the long-standing efforts in the diagnostic workup of rare diseases and a close collaboration between radiologists and oncologists.
