Abstract:

Ve své disertační práci jsem se věnoval obstrukční spánkové apnoe (OSA), charakteristice parametrů mikrocirkulace a specifikům ve vztahu k anestezii a intenzivní medicíně. OSA je často se vyskytující poruchou dýchání ve spánku a je spojena s kardiovaskulárními komorbiditami. Příčinou vzniku těchto komorbidit je nejpravděpodobněji porušená mikrocirkulace. Naší hypotézou bylo, že mikrocirkulace bude u pacientů s OSA porušená a závažnost tohoto postižení bude korelovat s tíží OSA. Do studie byli zařazeni pacienti přicházející k běžnému polysomnografickému vyšetření. K vyšetření mikrocirkulace byla použita technologie sidestream darkfield imaging. Do studie bylo zařazeno 33 konsekutivních pacientů. Pozorované změny mikrocirkulace mezi večerem a ránem byly vyjádřeny u pacientů s těžkou OSA a byly charakterizované snížením flow a zvýšením flow heterogenity. Je naší snahou upozornit na stoupající incidenci OSA, četnost a závažnost jejích komorbidit a možných důsledků tohoto syndromu pro perioperační období a pobyt na lůžku intenzivní péče. V neposlední řadě pak na důležitou úlohu anesteziologa v diagnostice a léčbě pacientů s OSA v perioperačním období.

Abstract:

In my doctoral thesis I focused on obstructive sleep apnea (OSA), characteristics of microcirculation parameters and specifics in relation to anesthesia and intensive medicine. OSA is the most common form of sleep disordered breathing and has been associated with major cardiovascular comorbidities. We hypothesized that the microcirculation is impaired in patients with OSA and that the magnitude of impairment correlates to OSA severity. Subjects were consecutive patients scheduled for routine diagnostic polysomnography. Sidestream darkfield imaging was used to assess the sublingual microcirculation. Thirty-three consecutive patients were included. Acute overnight microcirculatory changes are observed in subjects with severe OSA characterized by decreased flow and increased flow heterogeneity. It is our intention to highlight the increasing incidence of OSA, the frequency and severity of its comorbidities, and the possible consequences of this syndrome for the perioperative period and intensive care. Last but not least, to point out the important role of anesthesiologist in the diagnosis and treatment of patients with OSA in the perioperative period.
