Efekt rTMS na kognitivní funkce u pacientů s neurodegenerativním onemocněním mozku

The effect of rTMS on cognitive functions in patients with neurodegenerative diseases

Abstract:

Background. Repetitive transcranial magnetic stimulation (rTMS) is a promising tool to noninvesively study and modulate brain plasticity. Our aim was to investigate the effects of rTMS on cognitive functions in patients with Mild Cognitive Impairment (MCI), Alzheimer’s disease (AD) and Parkinson’s disease (PD). Methods. First of all we did complex review of literature subsequently pilot study on 10 MCI/AD patients. Twenty MCI/AD patients participated in the original study. Each patient received three sessions of 10 Hz rTMS of the right inferior frontal gyrus (IFG), the right superior temporal gyrus (STG), and the vertex (VTX, a control stimulation site) in a randomized order. Cognitive functions were tested prior to and immediately after each session. The GM volumetric data of patients were: 1) compared to healthy controls (HC) using source-based morphometry; 2) correlated with rTMS-induced cognitive improvement. Moreover, we studied subcortical-cortical rs-FC in PD or MCI-AD as compared to HC and the relation between FC abnormalities in patients and their cognitive outcomes. Results. High frequency rTMS of the right IFG induced significant improvement of attention and psychomotor speed (measured by TMT A and B) in patients with MCI/AD.The effect of the stimulated site on the difference in cognitive scores was statistically significant for the Word part of the Stroop test (ST-W, p = 0.012, linear mixed models). As compared to the VTX stimulation, patients significantly improved after both IFG and STG stimulation in this cognitive measure. MCI/AD patients had significant GM atrophy in characteristic brain regions as compared to HC (p = 0.029). The amount of atrophy correlated with the change in ST-W scores after rTMS of the STG. Striatal rs-FC was reduced in patients compared to HC. These changes were associated with motor and executive functions in PD and with memory in MCI-AD. Conclusion. rTMS enhanced cognitive functions in MCI/AD patients. We demonstrated for the first time that distinct pattern of GM atrophy in MCI/AD diminishes the cognitive effects induced by rTMS of the temporal neocortex. We also managed to find areas with decreased connectivity from the striatum in patients with PD and MCI/AD, which could be useful for subsequent stimulation with the effect on cognitive functions.

Úvod. Repetitivní transkraniální magnetická stimulace (rTMS) představuje hodnotný nástroj k neinvazivnímu studiu a modulaci mozku a mozkové plasticity. Naším cílem bylo prozkoumat efekt rTMS na kognitivní funkce u pacientů s mírnou kognitivní poruchou (MCI), Alzheimerovou chorobou (AD) a Parkinsonovou chorobou (PD). Metody. Nejdříve jsme provedli komplexní přehled literatury, pak publikovali pilotní studii na 10 MCI/AD pacientech. V originální práci bylo zařazeno dvacet pacientů s MCI/AD. Každý pacient absolvoval tři 10 Hz rTMS stimulace nad pravým gyrus frontalis inferior (IFG), pravým gyrus temporalis superior (STG) a nad vertexem (VTX, kontrolním místem stimulace) v náhodném pořadí. Před a po každé stimulaci byly testovány kognitivní funkce. Volumetrické data šedé hmoty MCI/AD pacientů byly: 1) porovnány s daty od zdravých kontrol (HC, healthy controls); 2) korelovány s rTMS-indukovaným kognitivním zlepšením. Dále jsme na určení dalších možných oblastí k následné stimulaci studovali subkortiko-kortikální funkční konektivitu (FC) u PD nebo MCI / AD ve srovnání s HC a vztah mezi změnami ve FC u pacientů a jejich kognitivním funkcemi. Výsledky. Vysokofrekvenční rTMS pravého IFG indukovala výrazné zlepšení pozornosti a psychomotorického tempa (měřeno pomocí TMT A a B) u pacientů s MCI/AD. Efekt místa stimulace na rozdíl v kognitivním skóre (před vs. po stimulaci) byl statisticky významný pro Stroop test, část Slova (ST-W, p = 0.012, s použitím lineárních smíšených modelů). V tomto testu se ve srovnání se stimulací nad VTX pacienti významně zlepšili po stimulací nad IFG a STG. MCI/AD pacienti měli v porovnání s HC významně více atrofie šedé hmoty v charakteristických oblastech mozku (p = 0.029). Míra atrofie korelovala se změnou skóre ST-W po rTMS nad STG. Striatální resting state FC byla snížena u pacientů ve srovnání s HC. Tyto změny byli spojené s motorickými a exekutivními funkcemi u PD a s pamětí u MCI-AD. Závěr. rTMS zlepšila kognitivní funkce u MCI/AD pacientů. Jako první jsme ukázali, že specifický vzorec atrofie šedé hmoty u MCI/AD pacientů snižuje efekt rTMS na kognitivní funkce při stimulaci nad temporálním neokortexem. Také se nám podařilo najít oblasti se sníženou konektivitou ze striata u pacientů s PD a MCI/AD, které by mohly být vhodné ke stimulaci a ovlivnění kognitivních funkcí.
