Long-term growth factors for cesarean deliveries

Abstract:

Tato diplomová práce se zaměřuje na problematiku porodů císařským řezem. I když tato metoda může zachránit život, je důležité, aby nebyla zneužívána a používána jen v opravdu nezbytných případech. Cílem práce je poskytnout ucelený přehled o nárůstu počtu porodů císařským řezem od 70. let a porovnat situaci v České a Slovenské republice. Zohledňuje faktory, které vedou k ukončení porodu císařským řezem, jak ze strany lékaře, tak ze strany rodičky. Metodika výzkumu zahrnovala kvantitativní a kvalitativní analýzu. Dotazníkové šetření mezi rodičkami a lékaři bylo prováděno v nemocnicích v Českých Budějovicích, Jindřichově Hradci a v Porodnici u Apolináře v Praze. Data byla zpracována do časových řad a vyhodnocena pomocí statistických metod. Zjištěné údaje ukazují, že věk rodičky a prvorodičky hrají klíčovou roli při volbě mezi porodem císařským řezem a přirozeným porodem. Nárůst porodů císařským řezem je patrný od 70. let až po současnost. Otázka VBAC (vaginální porod po císařském řezu) zůstává stále málo zastoupená. Je důležité, aby lékaři zohledňovali psychický stav rodičky, její zdravotní stav a její přání při rozhodování o metodě porodu. Závěrem lze konstatovat, že císařský řez by měl být používán jen v život ohrožujících situacích. Je třeba dbát na to, aby byl zohledněn celkový kontext a individuální potřeby rodičky.

Abstract:

This thesis deals with the problem of cesarean deliveries. Although this method can save lives, it is important that it is not misused and that it is used only in cases of genuine need. The aim of this thesis is to provide a comprehensive overview of the increasing number of cesarean deliveries since the 1970s and to compare the situation in the Czech Republic and Slovak Republic. It takes into account the factors that lead to the termination of the birth by cesarean deliveries, both by the doctor and by the mother. The research methodology included quantitative and qualitative analysis. A questionnaire survey of mothers and doctors was carried out in the hospitals in České Budějovice, Jindřichův Hradec and in the Apolinář Maternity Hospital in Prague. Data were processed into time series and evaluated using statistical methods. Findings show that the age of the mother and the primipara play a key role in the choice between cesarean delivery and natural childbirth. The increase in cesarean deliveries has been evident from the 1970's to the present. The question of VBAC (vaginal birth after cesarean) remains underrepresented. It is important that doctors take into account the psychological state of the mother, her medical condition, and her wishes when deciding on the method of delivery. In conclusion, cesarean deliveries should be used only in life-threatening situations. Care must be taken to ensure that the overall context and the individual needs of the mother is taken into account.
