Abstract:

Úvod. Pohybové intervenci jako součásti dialyzační léčby byla a je v praxi celosvětově věnována minimální pozornost. Bohužel i v ČR je velmi málo dialyzačních center, která by se touto problematikou zabývala. Zařazení fyzioterapie by však mohlo mít, dle mnoha studií, pozitivní vliv na progresi onemocnění, tělesnou zdatnost, svalovou sílu, bolest a celkovou kvalitu života nemocných v terminálním stádiu chronického onemocnění ledvin. Metody. Pacientům v ESRD účastnícím se naší studie byl jako součást léčby po dobu 20 týdnů zaveden řízený rehabilitační program (formou kinezioterapie nebo NMES mm. QF.). Na začátku a na konci programu byla provedena vyšetření k zhodnocení tělesné zdatnosti (spiroergometrie a šestiminutový test chůze), svalové síly (dynamometrie), kvality života (dotazník KDQOL), bolesti (Wong-Bakerův obličejový test) a arteriální tuhosti (CAVI). Statistické šetření bylo provedeno pomocí párového Wilcoxonova testu. Výsledky. Výsledky studie ukazují na velký pozitivní vliv fyzioterapeutických metod pro dialyzované pacienty. Pozitivní efekt na jednotlivé vyšetřované parametry jsme zaznamenali ve všech provedených testech dokonce se statistickou významností. Závěr. Na základě naší studie můžeme potvrdit, že fyzioterapie jako součást dialyzační léčby, může být pacientům velmi prospěšná a měla by jí být věnována dalekosáhlejší pozornost jak od odborné veřejnosti, tak samotných pacientů. Pacienti by měli mít přinejmenším možnost vybrat si, zda bude fyzioterapie součást jejich léčby. Fyzioterapeuti by měli působit ve všech dialyzačních centrech jako součást terapeutického týmu.

Abstract:

Background. Physical intervention as a part of dialysis treatment has been and is given minimal attention worldwide in practice. Unfortunately, even in the Czech Republic there are very few dialysis centers that would deal with this issue. However, the inclusion of physiotherapy could, according to many studies, have a positive effect on disease progression, physical fitness, muscle strength, pain and overall quality of life of patients with end-stage chronic kidney disease. Methods. A controlled rehabilitation program (in the form of kinesiotherapy or NMES mm. QF.) as part of treatment for 20 weeks was introduced to patients in the ESRD participating in our study. In the beginning and at the end of the program, examinations were performed to assess physical fitness (spiroergometry and a six-minute walk test), muscle strength (dynamometry), quality of life (KDQOL questionnaire), pain (Wong-Baker facial test), and arterial stiffness (CAVI). The statistical survey was performed using the paired Wilcoxon test. Results. The results of the study show a great positive effect of physiotherapeutic methods for dialysis patients. We recorded a positive effect on the individual examined parameters in all performed tests even with statistical significance. Conclusions. Based on our study, we can confirm that physiotherapy, as part of dialysis treatment, can be very beneficial for patients and should be given more far-reaching attention from both the professional community and the patients themselves. At the very least, patients should be able to choose whether physiotherapy is part of their treatment. Physiotherapists should work in all dialysis centers as part of the therapy team.
