Mobilization of hematopoietic stem cell progenitors and automated processing of grafts

Abstract:

Hematopoietic stem cell transplantation started more than 60 years ago and is now an essential part of many treatment concepts and protocols, not just in hematological malignancies. The whole process, starting from mobilization, collection and manipulation with cells, until freezing, storing and, finally, transplantation into patients, have been very well explored and described. However, there are still many problems and difficulties which continue to complicate this treatment protocol. These problems range from unsuccessful mobilizations, mostly in "poor mobilizers" or because of the treatment protocols in the induction regimen, to adverse events after transplantation, which could be partly caused by the cryoprotective agent dimethyl sulfoxide (DMSO). Although we have been aware of these problems for a long time, we haven't discovered adequate solutions for them, which means there still room for us to solve these issues. My thesis presents three publications, one published in a Czech journal and two in international journals. These publications explore the problematic fields of autologous hematopoietic stem cell transplantations. Our retrospective study of the newly used mobilization regimen with cytarabine (ARA-C) in our department shows its higher efficacy than the standard protocol, with satisfactory toxicity for the patients, and therefore, could partially offer a solution for problematic mobilizations. The second part of my work has been focused on toxicity of the cryoprotective solution DMSO. We started using an automated device for washing out DMSO before transplantation and analyzed the recovery of cells after the procedure, its impact on patients after infusion and verified safety for patients in terms of the time to hematopoietic recovery. After more than three years we evaluated all the results and confirmed that the washing procedure is effective, safe and has a positive contribution.

Transplantace krvetvorných buněk začaly před vice než 60-ti lety, a nyní jsou nezbytnou součástí léčebných postupů a protokolů nejen u hematologických malignit. Celý proces, od mobilizace, sběru a manipulace s buňkami až do jejich zamražení, skladování a nakonec transplantaci pacienta, jsou velmi dobře prozkoumané a popsané jevy. Nicméně, stále se zde nachází mnoho problémů a obtíží, které tento léčebný protokol komplikují. Od neúspěšné mobilizace, zejména u tzv. "poor mobilizers", nebo při použití některých léčebných protokolů v indukčním režimu, až po nežádoucí účinky po transplantaci, které mohou být částečně způsobeny dimethyl sulfoxidem (DMSO), kryoprotektivním činidlem. Tyto problémy jsou známy po dlouhou dobu, ale stále se nenašlo řešení, které by je vyřešilo. Proto je zde stále prostor na rozřešení těchto problému a nalezení odpovědi co za nimi stojí. Moje závěrečná práce zahrnuje tři publikace, jednu pro český časopis a dvě pro zahraniční časopisy, které zkoumají problematiku autologních transplantací krvetvorných buněk. Naše retrospektivní studie nově používaného mobilizačního režimu s cytarabinem (ARA-C) na našem oddělení, ukázala vyšší účinnost tohoto režimu než standardně používaného, s uspokojivou toxicitou pro pacienty, a může být tedy částečným řešením pro problematické mobilizace. Druhá část mé práce se zaměřila na toxicitu kryoprotektivního roztoku DMSO. Na našem pracovišti jsme začali používat automatický přístroj pro vymývání DMSO před transplantací, a analyzovali jsme zachování buněk po proceduře, vliv na pacienty po infuzi buněk a bezpečnost pro pacienty z hlediska obnovy krvetvorby po transplantaci. Po více než třech letech, jsme vyhodnotili výsledky, a potvrdili efektivitu promývacího postupu, bezpečnost a dobrý vliv pro pacienty.
