Malnutrition in hospitalized children

Abstract:

Cíl: Množství dětí přijímaných k hospitalizaci na dětská oddělení s malnutricí je podle různých prací odhadováno na 6 - 40%. Hlavním úkolem v diagnostice malnutrice je provádění screeningu nutričního rizika a identifikace pacientů vyžadujících nutriční podporu. Hlavním cílem studie bylo ověřit využitelnost testovacích škál k hodnocení rizika vzniku malnutrice u hospitalizovaných dětí. Pro hodnocení rizika vzniku dětské malnutrice byly použity dva hodnotící nástroje: První - škála Pediatrické skóre nutričního rizika a druhá škála - Screening Tool for the Assessment of Malnutrition in Paediatrics (STAMP). Metodika: Základní soubor 130 respondentů tvořilo 73 chlapců a 57 dívek ve věku 2 měsíce až 18 let, kteří byli hospitalizováni na Klinice dětského lékařství ve Fakultní nemocnici Ostrava. Výsledky: Po celkovém zhodnocení jednotlivých škál byl zjištěn statisticky významný rozdíl v detekování rizika vzniku podvýživy mezi škálami. Vyšší počet vysokého rizika vzniku malnutrice byl zjištěn škálou STAMP. Nebyl zjištěn rozdíl v riziku vzniku malnutrice mezi dětmi s odlišnými medicínskými diagnózami. U obou škál bylo také zjištěno, že není rozdíl mezi pohlavím a rizikem vzniku malnutrice. Závěr: Antropometrická hodnota, která u škály STAMP ovlivňuje celkové hodnocení rizika vzniku malnutrice, nemá velký význam v určení celkového nutričního rizika. U škály Pediatrického nutričního rizika jsou antropometrické hodnoty posuzovány v rámci vstupního formuláře. Celkové skóre nutričního rizika tyto antropometrické parametry neovlivňují, ale zásadně se uplatňují v doporučeních, které zahrnuje systém nutriční péče v nemocnici.

Abstract:

Objective: The perectage of children with malnutrition admitted to hospital for children, is 6 - 40% based on different studies. The main task in the diagnosis of malnutrition is a nutritional risk screening and identification of patients requiring nutritional support. The main objective of this study was to verify the usability testing of scales to assess the risk of malnutrition in hospitalized children. There was two evaluation tools used for risk of child malnutrition assessment: the first - Pediatric nutrition risk scale and the second - Screening Tool for the Assessment of Malnutrition in Paediatrics (STAMP). Methodology: The population of 130 respondents consisted of 73 boys and 57 girls in age from 2 months to 18 years who were hospitalized at the Department of Pediatrics at the University Hospital of Ostrava. Results: Was found statistically significant difference in detecting risk of malnutrition among the scales after the final assessment. A higher number of high risk of malnutrition was founded by STAMP scale. There was no difference in risk of malnutrition among children with different medical diagnoses. For both scales was also found that there is no difference between sex and risk of malnutrition. Conclusion: Anthropometric value that STAMP scale affects the range of an overall assessment of the malnutrition risk has no big importance in determining the overall nutritional risk. In the Pediatric nutrition risk scale anthropometric values are evaluated in the entry form. Anthropometric parameters doesn´t affect totals nutritional risks but the principles are applied in the recommendations which includes a system of nutritional care in hospital.
