Physiotherapy of a patient with polytrauma in intensive care

Abstract:

Tato bakalářská práce se věnuje fyzioterapii polytraumatizovaného pacienta. Cíl práce: Cílem bakalářské práce je přispět do problematiky fyzioterapie u pacientů po polytraumatu v intenzivní péči pomocí vhodně zvolených způsobů léčby. Metodika: V této práci je zpracována kazuistika pacienta po polytraumatu u kterého byl proveden vstupní kineziologický rozbor, na jehož základě byl vypracován krátkodobý fyzioterapeutický plán. Vzhledem k výsledkům jsem využila techniky měkkých tkání k ošetření jizev a fascií. Ke zvětšení rozsahu pohybů v kloubech, zvýšení svalové síly bylo použito analytického cvičení a konceptu proprioceptivní neuromuskulární facilitace (PNF). Pomocí respirační fyzioterapie byl ovlivněn stereotyp dýchání a pohyb hrudníku. Realizace plánu byla uskutečněna ve 12 jednotlivých terapiích v délce trvání 9 týdnů. Po výstupním kineziologickém rozboru byl stanoven dlouhodobý plán fyzioterapie. Výsledky: U pacienta došlo k zlepšení stereotypu dýchání, ke zvýšení svalové síly všech končetin, zvětšení rozsahu pohybů, a to zejména v oblasti ramenního kloubu pravé horní končetiny (PHK). Během terapeutických sezení byl pacient postupně vertikalizován. Závěr: Na základě vhodně zvolených fyzioterapeutických postupů a metod došlo ke splnění cílů, které byly realizovány v rámci krátkodobého fyzioterapeutického plánu.

Abstract:

This bachelor´s thesis is devoted to the physiotherapy of a polytraumatized patient. Introduction: Physiotherapy in a patient with polytrauma in intensive care. Objective: The aim of the bachelor's thesis is to contribute to the issue of physiotherapy for patients after polytrauma in intensive care using appropriately chosen methods of treatment. Methodology: This work presents a case study of a patient after polytrauma who underwent an initial kinesiology analysis, on the basis of which a short-term physiotherapy plan was developed. Due to the results, I used soft tissue techniques to treat scars and fascia. Analytical exercises and the concept of proprioceptive neuromuscular facilitation (PNF) were used to increase the range of motion in the joints and increase muscle strength. Using respiratory physiotherapy, the breathing stereotype and chest movement were affected. The plan was implemented in 12 individual therapies lasting 9 weeks. After the final kinesiology analysis, a long-term physiotherapy plan was established. Results: The patient's breathing pattern improved, the muscle strength of all limbs increased, and the range of motion increased, especially in the area of the shoulder joint of the right upper limb. During the therapy sessions, the patient was gradually verticalized. Conclusion: On the basis of appropriately chosen physiotherapy procedures and methods, the goals that were implemented as part of the short-term physiotherapy plan were met.
