Abstract:

Disertační práce předkládá postup vyhledávání a statistické evaluace laboratorních parametrů jako potenciálních lékově specifických rutinně analyzovaných biomarkerů v onkologické léčbě. V úvodní části práce, navazující na předchozí výzkumnou čin-nost autora, je popsán význam terapeutického monitorování léčiv, hledání nových biomarkerů toxicity a léčebné odpovědi. Teoretická část popisuje podrobně indikace, farmakodynamický účinek sunitinibu a jeho vztah k léčebné odpovědi i nežádoucím účinkům na molekulární úrovni a farmakokinetiku léčiva. Metodická část práce představuje explorativní přístup k analýze dynamiky jednotlivých parametrů, de-tailně popisuje statistický přístup k hodnocení dynamiky vybraného laboratorního parametru středního objemu erytrocytů, laboratorní i klinické hodnocení. Hlavním výstupem práce je zjištěná statisticky významná korelace rychlosti nárůstu středního objemu erytrocytů s klinickou toxicitou sunitinibu u dospělých pacientů. Dále byl v této práci poprvé potvrzen jev sunitinibem-indukované elevace středního objemu erytrocytů u dětských onkologických pacientů, u kterých je přítomen i v případě současně podávané konvenční chemoterapie. Práce identifikuje tento parametr jako vhodný pro další prospektivní výzkum u dětských pacientů, léčených tyrosin-kinázovými inhibitory, potenciálně ovlivňujícími jeho dynamiku.

Abstract:

This dissertation introduces scientific approach to identification and statistical eval-uation of possible drug-specific routinely assayed laboratory biomarkers in anti-cancer chemotherapy. Introduction part of the dissertation follows up authors pre-vious research and describes the importance of the therapeutic drug monitoring and investigation of new biomarkers of toxicity and therapeutic response. The theo-retical part describes main sunitinib indications, its pharmacodynamic relation to both therapeutic response and adverse events at a molecular level and pharmacoki-netics. The methodological part presents an explorative approach to evaluation of monitored laboratory parameters, describes the statistical evaluation of selected mean corpuscular volume as a candidate therapeutic biomarker. The main result of the dissertation is a finding of statistical significance observed between the rate of sunitinib-induced mean corpuscular volume elevation and clinical toxicity in adult patients. Moreover, the thesis is first to describe the above phenomenon in paediat-ric patients receiving concomitant conventional chemotherapy. The thesis identifies mean corpuscular volume as a candidate marker for further evaluation in paediatric patients receiving tyrosin-kinase inhibitors affecting its dynamics.
