Mental Health Care for Adults With Dual Diagnosis

Abstract:

Diplomová práce se zabývá tématem odborné péče o duševní zdraví v životě dospělých osob s mentálním postižením a přidruženou psychickou poruchou. Cílem práce je zjistit, jak osoby s lehkým mentálním postižením a přidruženou psychickou poruchou a o ně pečující osoby vnímají poskytovanou psychiatrickou a psychologickou péči, jak tuto péči hodnotí a jaké s ní mají zkušenosti. Realizovaný kvalitativní výzkum vychází z modelů kvality péče z pacientovy perspektivy (např. Larsson, & Larsson, 2001; Sixma, et al., 1998). Na základě analýzy 8 rozhovorů s lidmi s mentálním postižením a 8 rozhovorů s pečujícími osobami bylo zjištěno, že osoby s mentálním postižením i pečující osoby jsou důležitými aktéry v poskytované péči a že péče o duševní zdraví zastává důležitou roli v systému sociální opory osob s mentálním postižením a přidruženou psychickou poruchou. Obě skupiny respondentů zdůrazňují při hodnocení péče interpersonální a komunikační aspekty a vyjadřují větší spokojenost s psychologickou než s psychiatrickou péčí, přičemž osoby pečující jsou při svém hodnocení kritičtější. Analýza percepcí respondentů umožnila identifikovat specifické potřeby, které lidé s mentálním postižením ve vztahu k poskytované péči o duševní zdraví mají, a to zejména potřebu bližšího vzájemného vztahu s odborníky, potřebu kvalitní komunikace a potřebu intervence přizpůsobené specifikům vyplývajícím z mentálního postižení.

Abstract:

The diploma thesis is concerned with the topic of specialist mental health care for adults with dual diagnosis. The aim of the thesis is to find out how people with dual diagnosis and their caregivers perceive provided psychiatric and psychological care, how they evaluate the care and what experiences they have with it. The realized qualitative research is based on models of quality of care viewed from the patient´s point of view (e.g. Larsson, & Larsson, 2001; Sixma, et al., 1998). Following the analysis of eight interviews with people with learning disabilities and eight interviews with caregivers it was found out that people with learning disabilities and also caregivers play an important role in the provided care, and that mental health care is a very important part of the system of social support for people with dual diagnosis. In the evaluations of care both groups of respondents emphasize interpersonal and communication aspects and they express greater satisfaction with provided psychological care than with psychiatric care. The caregivers are more critical in their evaluations. The analysis of the respondents´ perceptions enabled to identify specific needs of people with learning disabilities in relation to provided mental health care, particularly a need of close relationship with specialists, a need of quality communication and a need of intervention adapted to specifics which are consequential to learning disabilities.
