Physiotherapy after spondylolisthesis spine surgery

Abstract:

Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacientů po operaci spondylolistézy. Metodika: U pacientky s degenerativní spondylolistézou byl proveden vstupní kineziologický rozbor (statické a dynamické vyšetření bederní páteře, vyšetření posturální stability dle Koláře) s nálezem dysfunkce HSS, patologický nález hodnot v rámci dynamického vyšetření páteře, oslabení svalové síly m. quadriceps LDK, bolestivost bederní páteře s iradiací do LDK, omezení ADL aktivit. Krátkodobý plán rehabilitace byl sestaven na základě mých osobních zkušeností a z výše popsaných výsledků vstupního vyšetření. Na období časné pooperační rehabilitace jsem zvolila metody respirační fyzioterapie, CG, lehkého kondičního cvičení. Postupně, dle zlepšujícího se stavu pacientky, jsem přidávala prvky z neurodynamiky, Brunkow apod. Vertikalizace byla povolena druhý pooperační den zpočátku ve vysokém chodítku, postupně pacientka zvládala chůzi bez kompenzačních pomůcek. Před propuštěním byla provedena instruktáž režimových opatření, pacientka byla poučena o důležitosti následného cvičení a informována ohledně změny pohybových stereotypů. Výsledky: U pacientky došlo po půl roce od operace k vymizení bolestí do LDK, zmírnily se bolesti v bederní páteři. Došlo ke zlepšení v oblasti ADL, zlepšení kvality života v oblasti osobní péče, sezení, spaní, cestování. Zlepšila se svalová síla m. quadriceps LDK (z původních 3-4 na 4-5 dle ST dle Jandy), stejně tak i naměřené hodnoty v rámci dynamického vyšetření bederní páteře spadaly do fyziologických hodnot. Přetrvávají asymetrie obvodů levého stehna a pravého lýtka, které jsou menší o 1cm, stejně jako u vstupního vyšetření. Mírného zlepšení bylo dosaženo i u kontrolního vyšetření posturální stabilizace dle Koláře. Zde má pacientka ještě rezervy a bude vhodné ji motivovat k dalšímu cvičení do budoucna.

Abstract:

Aim of the Thesis: The aim of the Thesis is the contribution for solution on the physiotherapy matters for patients after operation of the spondylolisthesis. Methodology: There was an initial kinesiology analysis (static and dynamic examination of the lumbar spine, examination of the postural stability according to Kolář) carried out for the woman patient with degenerative spondylolisthesis with the findings of the malfunction of the deep stabilization system, pathological findings of the values within the dynamic spine examination, weakening of the muscle force for m. quadriceps left leg, painfulness of the lumbar spine with irradiation into the left leg, limiting of the ADL activities. Short-term rehabilitation schedule was established based on my personal experience and above described results of the initial examination. For the period of the early past-operation rehabilitation I chose methods of respiration physiotherapy, vascular training, moderate fitness exercise. Gradually according to the improvements of the patient condition I added neuro-dynamic elements, Brunkow etc. Verticalisation was allowed on the second past-operation day, with the walker first, gradually the patient was able to walk without any compensatory aids being used. Before releasing the training for regime arrangement was carried out, the patient was instructed on the importance of follow-up exercising and informed about change of the movement stereotypes. Results: After half year from the operation of the patient there was an elimination of pain in left leg and the pain in lumbar spinal area reduced. Improvement in the area of ADL took place, improvement of quality of life at the area of personal care, sitting, sleeping, travelling. Improvement of the muscle force for m. quadriceps left leg (from initial 3 - 4 to 4 - 5 according to Janda), as well as the measured values in the area of dynamic examination for the lumbar spine fell into the physiologic values. Asymmetry of the girth for left thigh and right calf remaining which are 1cm smaller, as well as with the initial examination. Slight improvement was achieved also at the control examination of the postural stability according to Kolář. There is still a space for improvement and it is still appropriate to motivate the patient for further exercising.
