Principal-agent problem, application to health care in Ústecký kraj

Abstract:

České zdravotnictví dlouhodobě čelí kritice ohledně financování i poskytované zdravotní péče. Kritika zdravotnictví je v Ústeckém kraji umocněna vnikem Krajské zdravotní a.s., která sdružuje 5 nemocnic v Ústeckém kraji a tyto centrálně řídí. Krajská zdravotní vznikla za účelem zlepšení kvality a zvýšení efektivity poskytované zdravotní péče. V první části práce jsou vymezeny základní pojmy ve zdravotnictví a základní specifika Ústeckého kraje. Dále je v práci zasazeno zdraví do rámce ekonomické teorie, stejně tak sektory zdravotnictví a následně je konfrontována situace v Ústeckém kraji rovněž s ekonomickou teorií. Rovněž se práce věnuje společenskému a politickému vnímání zdravotnictví. V druhé části práce měří kvalitu poskytované zdravotní péče v Ústeckém kraji pomocí počtu zdravotních výkonů jednotlivých nemocnic a jejich spádových oblastí s přihlédnutím k tomu, zda je nemocnice samosprávná, nebo zda spadá pod sdružení Krajské zdravotní a.s. V této práci jsou pak konfrontovány tyto dva typy nemocnic. Zvolená hypotéza byla formulována takto: "Nemocnice spadající pod Krajskou zdravotní a.s. poskytují méně kvalitní zdravotní péči". Hypotéza byla v této práci potvrzena, ale zároveň byly modelu vytknuty slabiny a v návaznosti na to byla specifikována možná vylepšení výzkumu, prostor pro navázání podobných prací a doporučení, která by mohla vést k vylepšení poskytované zdravotní péče v Ústeckém kraji.

Abstract:

Czech health system has been for a long time criticised both for financial side and for the care provided. In the Ústí nad Labem Region the criticism has been intensified by the establishment of the joint-stock company Krajská zdravotní that manages five hospitals there. Krajská zdravotní was established so that care could be provided with greater efficiency. The first part of this paper delineates basic terms of healthcare and some specific aspects of the Region. The paper continues to put first health and different branches of the healthcare system in context of economic theory, later the situation in the Ústí nad Labem Region is put in the theoretical context as well. Social and political perception of the healthcare system is also taken into account. In its second part the paper aims to assess the quality of provided care using the number of medical procedures related to the catchment area of respective hospitals. Attention is given to whether a hospital is managed by Krajská zdravotní or independently. The two types of hospitals are then contrasted. The hypothesis is as follows: "Hospitals managed by Krajská zdravotní provide lower quality healthcare." The research confirmed the hypothesis but at the same time certain weak points in the model were found. Specific paths to achieving a greater perfection both in future research and in healthcare system in the Region are suggested.
