Risk of falls in older adults after operation at the orthopedic department

Abstract:

Cíl: Cílem práce je zjistit výskyt rizika pádů u seniorů nad 65 let po operaci na ortopedickém oddělení Pardubické krajské nemocnice (PKN) a.s. Metodika: Do souboru bylo zařazeno 100 pacientů po ortopedické operaci hospitalizovaných v PKN a.s. starších 65 let. Data byla získána ze zdravotnické dokumentace či přímo od pacienta a vyplňována do již připravených dotazníku. Dotazník se skládá z demografických údajů a tří screeningových testů hodnotících riziko pádu ? škála Morseové (Morse Fall Scale), Conleyové modifikované Juráskovou 2006 a hodnotící škála používaná v PKN a.s. Sběr dat probíhal od září 2013 do února 2014 na ortopedickém oddělení PKN a.s. se souhlasem pracovníka centra vzdělávání a vrchní sestry ortopedického oddělení. Data byla statisticky zpracována pomocí ?2 testu na hladině významnosti 5 % (? = 0, 05) a pomocí popisné statistiky. Výsledky: Byl zjištěn rozdíl ve validitě použitých screeningových testů k hodnocení rizika pádu. Dosažené výsledky identifikovaly senzitivitu škály Juráskové 37 %, PKN 47 % a MFS 100 %. Specificita škály Juráskové je 98 %, PKN 98 % a MFS 30 %. Nebyl zjištěn významný rozdíl rizika pádu v závislosti na pohlaví ani ortopedické diagnóze. Vliv na riziko pádu má porucha kognitivních funkcí, užívání rizikových léků, smyslový deficit a přítomnost vedlejší diagnózy. Závěr: V našem hodnocení vyšli více rizikoví senioři nad 80 let u jednoho hodnotícího testu ze tří používaných. Každý screeningový test se ve svém hodnocení liší. Proto je vhodné dále posuzovat validitu používaných testů a sjednotit se v názorech na aplikaci nejvhodnějšího nástroje hodnotícího riziko pádu u seniorů v praxi.

Abstract:

Aim: The aim of this thesis is to determinate the risk of falls of seniors after an operation at the orthopaedic department of the Regional Hospital of Pardubice a.s. Methodology: The file contained 100 patients with the age over 65 years hospitalized at the orthopaedic department in the Regional Hospital of Pardubice a.s. The data were collected from medical documentation or directly from the patient, and were filled into the prepared questionnaires. The questionnaire consists of demographics data and three screening tools for determining the risk of fall ? Morse Fall Scale, the scale of Conley modified by Jurásková 2006, and the scale used in the Regional Hospital of Pardubice a.s. (PKN). The data were obtained from September 2013 to February 2014 at the orthopaedic department of the Regional Hospital of Pardubice a.s. with the consent of employees (an education centre officer, a clinical nurse manager). Gathered data were processed statistically in analytical charts, compiled using the statistical test ?2 with the level of significance 5 % (? = 0, 05), and with the help of descriptive statistics. Results: There is a difference in validity of the used screening tools for determination of the risk of fall. The results obtained identified the sensitivity of Jurásková Scale of 37 %, PKN of 47 %, and MFS of 100 %. The specificity of Jurásková Scale is 98 %, of PKN is 98 % as well, and of MFS is 30 %. There is no gender- and orthopaedic diagnosis-related difference in the risk of fall. The influences on the risk of fall have disorders of cognitive functions, the usage of high-risk medicaments, sensory deficiency, and the presence of secondary diagnosis. Conclusion: Based on the results of one screening tool out of the three used, the risk of fall is higher for patients over 80 years. Every tool is different. Therefore, it is appropriate to assess the validity of the tools used, and unify in practice the standpoints on the application of the best tool for the assessment of the risk of falls of seniors.
