Nursing Care of Patient with Acute Psychotic Disorders

Abstract:

Téma bakalářské práce je zaměřeno na akutní a přechodnou psychotickou poruchu a psychiatrickou ošetřovatelskou péči. Cílem bakalářské práce je popsat a analyzovat ošetřovatelskou péči o 40letou pacientku s lékařskou diagnózou akutní a přechodné psychotické poruchy (F23) – akutní polymorfní psychotická porucha bez schizofrenních symptomů (F23.0), realizovanou metodou ošetřovatelského procesu a popsanou ve všech jeho pěti fázích. Do kazuistiky je vybrán případ pacientky s akutní polymorfní psychotickou poruchou bez schizofrenních symptomů, nedobrovolně hospitalizované na akutním psychiatrickém oddělení zvýšené akutní péče z důvodu observace a stabilizace jejího psychického stavu s následným překladem na terapeutické oddělení. Posouzení zdravotního stavu pacientky je realizováno podle oblastí uvedených v NANDA doménách. Na základě zjištěných ošetřovatelských problémů jsou stanoveny ošetřovatelské diagnózy, které umožňují naplánovat cíle a intervence ošetřovatelské péče. Detailně je popsáno dynamické hodnocení situační agrese DASA-IV, které se používá jen na akutním oddělení, kde byla pacientka hospitalizována, riziko násilí vůči druhým a následné vyhodnocení tohoto rizika. Výstupem bakalářské práce jsou doporučení pro praxi vycházející z kazuistiky pacientky. Kazuistika je zajímavá tím, že diagnóza nebyla úplně jednoznačná od samého počátku hospitalizace pacientky, neboť často se konečná diagnóza stanovuje v průběhu léčby z důvodu variabilních symptomů duševní poruchy, čemuž také musí odpovídat poskytnutá ošetřovatelská péče.

Abstract:

The topic of the thesis is focused on acute and transient psychotic disorder and psychiatric nursing care. The aim of the thesis is to describe and analyze nursing care for a 40-year-old female patient with a medical diagnosis of acute and transient psychotic disorders (F23) – acute polymorphic psychotic disorder without schizophrenic symptoms (F23.0), implemented using the nursing process method and described in all five phases. The patient selected for the case study is one with an acute polymorphic psychotic disorder without schizophrenic symptoms, involuntarily hospitalized in an acute psychiatric care ward for observation and stabilization of her mental state, followed by transfer to a therapeutic ward. The assessment of the health status of the patient is conducted according to the areas specified in the NANDA domains. Based on identified nursing problems, nursing diagnoses are established, enabling the planning of nursing care goals and interventions. Dynamic assessment of situational aggression using DASA-IV is described in detail, which is used only in the acute unit where the patient was hospitalized, assessing the risk of violence towards others and subsequent evaluation of this risk. The output of the thesis includes recommendations for practice based on the case study of the patient. The case study is interesting because the diagnosis was not entirely clear from the beginning of the hospitalization of the patient, as the final diagnosis is often determined during treatment due to the variable symptoms of mental disorder, which must also be reflected in the provided nursing care
