Neural substrates of social cognition in schizophrenia

Abstract:

Úvod: Těžkosti v sociální kognici představují jeden z hlavních deficitů u schizofrenie. Pacienti se schizofrenií vykazují těžkosti v různých aspektech sociální kognice, včetně teorie mysli, emočních procesů jako rekognice a uvědomění a zároveň mají narušené zpracování figurativního jazyka, například ironie nebo humoru. Nejprve prezentujeme meta-analýzu funkčních studií abychom vymezili společné substráty kognitivních a afektivních aspektů narušené sociální kognice u schizofrenie. Dále se zabýváme emočním uvědoměním a jeho strukturálními koreláty u schizofrenie. Nakonec zkoumáme asociaci mezi neurálními substráty humoru u schizofrenie na jedné straně a psychopatologií s kognicí na straně druhé. Metody: Nejdřív jsme použili meta-analytickou metodu, abychom prozkoumali aktivaci mozku během emoční rekognice z tváří (12 studií) a teorií mysli (16 studií) u pacientů se schizofrenií a zdravých kontrol a identifikovali tak jedinečné a společné regiony. Dále jsme extrahovali objemy šedé hmoty ve vybraných regionech síti emočního uvědomění u 29 pacientů se schizofrenií a 33 zdravých kontrol. Objemy těchto regionů jsme porovnali mezi skupinami a dále korelovali se Škálou úrovní emočního uvědomění. Nakonec jsme měřili mozkovou aktivitu pacientů se schizofrenií, kdy 25 podstoupilo fMRI proceduru a 40 podstoupilo EEG proceduru, přičemž plnili úkol na porozumění humoru. Symptomy a kognici jsme korelovali s aktivitou v rámci regionů zpracovávajících humor pomocí fMRI a efektivní konektivitu mezi těmi samými regiony pomocí metody EEG-Directed transfer function. Výsledky: Výsledky meta-analýzy ukazují jak specifické, tak společné funkční abnormality související s emočními i kognitivními aspekty sociální kognice u schizofrenie. Překryv obou modalit byl nalezen v pravé insule, anteriorním cingulu mediálním prefrontálním kortexu a bilaterálně v inferiorním parietálním kortexu. Výsledky volumetrické studie naznačují souvislost mezi objemem šedé hmoty pravého precuneu a deficitem v uvědomování si emočních stavů druhých u schizofrenie. Výsledky naší třetí studie ukazují, že intenzita zpracování humoru u schizofrenie souvisí s úrovní kognitivních funkcí a taky se závažností schizofrenní psychopatologie, co se promítá do změněné aktivace a efektivní konektivity ve fronto-temporo-parietální síti. Závěr: Demonstrovali jsme komplexní narušení sociální kognice u schizofrenie napříč různými modalitami. Schizofrenie je charakterizovaná narušeními neurálních okruhů u afektivní i kognitivní složky sociálních procesů. Deficity v afektivní modalitě byly asociovány se strukturními změnami specifickými pro schizofrenii. Narušená sociální kognice u schizofrenie se taktéž projevuje v problémech s porozuměním humoru, což se odráží v změněné konektivitě uvnitř sociálně-kognitivní síti a je asociováno se schizofrenní symptomatologii.

Abstract:

Background: Deficits in social cognition constitute one of the core features of schizophrenia (SZ). Patients with SZ manifest difficulties in various aspects of social cognition, including theory of mind, emotional processing including recognition and awareness as well as processing figurative language such as irony and humor. Here, we first present a meta-analysis of functional studies to delineate common underpinnings of cognitive and affective aspects of impaired social cognition in SZ. Further we focus on emotional awareness and its structural correlates in SZ. Finally, we investigate the associations between neural substrates of humor processing and psychopathology and cognition in SZ. Methods: First, we used meta-analytic method to explore brain activation during facial emotion recognition (12 studies) and theory of mind tasks (16 studies) in SZ patients and healthy controls and identify unique and overlapping regions. Second, we extracted grey matter volumes of emotional awareness network in 29 SZ patients and 33 healthy controls. The volumes of these regions were compared between groups as well as further correlated with Levels of emotional awareness scale. Third, we assessed 25 SZ outpatients in an fMRI procedure and 40 in an EEG procedure while they completed humor comprehension task. The symptom severity and cognition were correlated with activation within the humor processing regions using fMRI and effective connectivity using an EEG-based Directed transfer function (EEG-DTF) method. Results: The meta-analytic results show specific as well as common functional alterations related to emotional as well as cognitive aspects of social cognition in SZ. The overlap of both modalities was found in right insula, anterior cingulate, and medial prefrontal cortex and increased activation in the bilateral parietal cortex. The findings of the volumetric study suggest a relationship between gray matter volume of the right precuneus and deficits in understanding of emotional states of others in SZ. Finally, the results of our third study show the intensity of humor processing (fMRI) in schizophrenia is related to the level of cognitive functioning and well as to the severity of schizophrenia psychopathology that is also reflected in altered activation and effective connectivity (EEG-DTF) in the fronto-temporo-parietal network. Conclusions: We demonstrated complex impairment of social cognition in SZ across different modalities. In conclusion, SZ is characterized by wide neural-related circuit disruptions during both affective and cognitive social processing. Deficits in affective modality were associated with structural changes specific to SZ. The deficient social cognition in SZ is also manifested in problems with humor understanding, which is reflected in altered connectivity between social-cognitive network and associated with SZ symptoms.
