Evaluating the effect of therapy in patients with Parkinson's disease using functional Magnetic Resonance Imaging: pharmaco-fMRI

Anotace:

U pacientů s Parkinsonovou nemocí (PN) se vyskytuje porucha řeči typu hypokinetické dysartrie, která často nereaguje na dopaminergní medikaci. U některých pacientů se porucha řeči s postupujícím onemocněním zhoršuje. Mnoho publikovaných studií se zabývalo vlivem medikace na změny parametrů řeči, ale výsledky nejsou konzistentní. Neurální koreláty hypokinetické dysartrie také nejsou přesně známy. Zjišťovali jsme, zda výchozí akustické parametry mohou předpovědět průběh PN, s ohledem na kognitivní funkce pacientů. Jen málo studií se přímo zabývalo účinkem dopaminergní medikace pomocí fMRI, aby se zjistilo, zda a jak tato medikace ovlivňuje plasticitu mozku v artikulačních sítích během řečového úkolu a v klidu. Hlavním cílem této studie bylo prozkoumat vliv dopaminergní medikace na motorické řečové sítě u chronicky léčených pacientů s pokročilou PN a posoudit souvislost s akustickými parametry jejich hlasu a řeči.

Abstract:

Patients with Parkinson’s disease (PD) experience a speech disorder called hypokinetic dysarthria that is often unresponsive to dopaminergic therapy. Over the course of the disease, the speech disorder worsens in some patients. Many published studies have examined the effect of medication on changes in speech parameters, but the results are not consistent. The neural correlates of hypokinetic dysarthria are also not precisely known. We investigated whether baseline acoustic parameters can predict the course of PD, and we considered patient cognition. Few studies have directly addressed the effect of dopaminergic medication using fMRI to explore whether and how this medication affects brain plasticity in articulatory networks during a speech task and at rest. The main aim of this study was to investigate the effect of dopaminergic medication on motor speech networks in chronically treated patients with longstanding PD and to assess the association with the acoustic parameters of their voice and speech.
