Physiotherapy of painful shoulder syndrome

Abstract:

Cíl: Cílem práce je přispět k řešení problematiky volby vhodných fyzioterapeutických postupů, metod a konceptů u pacienta s diagnózou syndrom bolestivého ramene tendinitis m. bicipitis brachii. Metodika: Zpracovala jsem kazuistiku pětadvacetiletého muže s diagnózou tendinitis m. bicipitis brachii. Provedla jsem vstupní kineziologické vyšetření a na jeho základě navrhla krátkodobý fyzioterapeutický plán, jeho realizace trvala celkem 7 týdnů, během kterých proběhlo 10 jednotlivých sezení. V rámci plánu byly využity následující fyzioterapeutické metody a koncepty: cvičení v uzavřených kinematických řetězcích, kineziotaping, Agisticko-excentrické kontrakční postupy, Dynamická neuromuskulární stabilizace, Spiraldynamik?, Brüggerův koncept, cvičení s thera-bandem, Feldenkraisova metoda, Bazální posturální programy - fyzioterapeutický koncept podle Čápové. Důležitou součástí terapie byla také edukace pacienta. Výsledky: Podařilo se odstranit bolest, obnovit fyziologický rozsah pohybu, protáhnout zkrácené svaly, zmírnit svalové dysbalance, zlepšit dechový stereotyp, funkci hlubokého stabilizačního systému a vnímání vlastního těla. Závěr: Dosažené výsledky svědčí účinnosti zvolených metod a konceptů fyzioterapie využitých v rámci realizace krátkého fyzioterapeutického plánu. Svou zásluhu nese také velká motivace pacienta, který výborně spolupracoval.

Abstract:

The title: Physiotherapy of painfull shoulder syndrome The goal: The goal of my thesis is contribute to solution of problems at syndrome of painfull shoulder-tendinitis of bicipiti brachii muscle, in a way of finding the appropriate physiotherapetic procedures, methods and concepts. The methodology: I elaborated the casuistic of 25 years old man with dignosis of bicipiti brachii muscle's tendinitis. I made an initial kinesiology examination and I suggested a short-term physiotherapeutic plan on its basic. The short-term plan's realization took 7 weeks in total and included 10 sessions. The used methods and concepts were: closed chain exercises, kinesiotaping, agistic-eccentric contraction exercises, dynamic neuromuscular stabilization, Spiraldynamik?, Brügger's concept, Thera-Band Exercise, Feldenkrais Method, Basic postural programs - Physiotherapeutic Concept according to Čápová. Another important part of the therapy was to educate the patient. Results: I succesfully removed the pain, restored the physiological range of movement, reduced the muscle contraction and muscle imbalance, improved breathing stereotype and the deep stabilisation system and I also improved the patient's perception of his own body. Conclusion: The achieved outcome shows effectivity of selected methods and concepts what were used in the short-term physiotherapy plan. Patient's motivation and good cooperation also influenced the effectivity of the therapy.
