The problem of the anterior cruciate knee ligament in selected sports (ice hockey, volleyball, football)

Abstract:

Poranění předního zkříženého vazu je velmi častým poraněním v oblasti kolenního kloubu. Mnohým sportovním zraněním se dá předcházet vhodnou prevencí, jako je nácvik správné techniky pro daný sport, volba vhodných ochranných pomůcek, dostatečná regenerace atd. Většina poranění předního zkříženého vazu vzniká podobným pro daný sport charakteristickým mechanismem. V ledním hokeji jde nejčastěji o přímý kontakt "koleno na koleno``, ve volejbale o nekoordinovaný doskok u sítě a u fotbalistů se tomu děje nejčastěji při dopadu po hlavičkovém souboji nebo při rotaci kolem stojné nohy. Následuje terapie buď konzervativní, či chirurgická. Při operační léčbě se odebírají štěpy z lig. patellae, hamstringů, či z m. rectus femoris. Na výběr jsou i kadaverozní štěpy a syntetické materiály. Nedílnou součástí terapie je následná rehablitace, která je ovšem u sportovců často akcelerovaná, zaměřená zejména na charakteristické svalové skupiny důležité pro daný sport a nezastupitelnou roli hraje také psychická stránka a adherence pacienta k terapii

Abstract:

Damage to the anterior cruciate ligament is a very frequent injury in the knee joint region. Some sport injuries can be avoided by appropriate prevention, such as the practice of the correct technique specific for the given sport, the selection of suitable equipment, satisfactory regeneration, etc. Most injuries to the anterior cruciate ligament develop by a similar mechanism characteristic of the given sport. In ice hockey, it is, most often, a direct contact ``knee to knee", in volleyball it is the uncoordinated landing at the net, and in footballers, this happens most frequently on landing after a head-to-head duel or upon rotation around the standing leg. The subsequent therapy is either conservative or surgical. In the surgical treatment, grafts from the patellar tendon, from the hamstrings or from the rectus femoris muscle are taken. Also, cadaverous grafts and synthetic materials are available. An integral part of the therapy is the subsequent rehabilitation, which is often accelerated in the sportsmen, focused on characteristic muscle groups important for the given sport; the mental aspect also plays an essential role here, as well as the patient's adherence to the therapy
