The significance of the involvement of small nerve fibers in the pathophysiology of paiful diabetic neuropathy

Abstract:

Diabetes mellitus 2. typu (DM2) je chronické onemocnění, jehož prevalence v posledních letech výrazně vzrůstá. Prezentovaný soubor prací poskytuje komplexní prospektivní zhodnocení souboru pacientů s časnými stádii DM2 a prediabetu a jeho srovnání s kontrolním souborem zdravých normoglykemických dobrovolníků. Nejpřínosnějším aspektem této práce je precizní a velmi detailní vyšetření jasně definovaného souboru pacientů všemi stěžejními dostupnými klinickými i laboratorními testy užívanými v diagnostice periferních neuropatií. Kromě velmi podrobného klinického vyšetření absolvovali všichni jedinci také kondukční studie a elektromyografické vyšetření, somatosenzitivní evokované potenciály (zejména k vyloučení event. koincidence subklinické centrální léze somatosenzitivní dráhy), kvantitativní testování senzitivity, vyšetření spektrální analýzy variability srdeční frekvence a v neposlední řadě i odběr kožní biopsie se stanovením intraepidermální hustoty nervových vláken (IENFD) pomocí panaxonálního markeru protein gene product 9.5 (PGP9.5), jakožto nejdůležitějšího parametru kožních biopsií. Součástí komplexního zhodnocení dat získaných ze souboru zdravých dobrovolníků bylo také stanovení vlastních validních věkově stratifikovaných normativních dat u vybraných diagnostických metod (kožní biopsie, kvantitativní testování senzitivity, spektrální analýza variability srdeční frekvence). Ve shodě s již publikovanými výsledky byl potvrzen signifikantní vliv věku a méně významný vliv pohlaví zdravých dobrovolníků u všech vyšetřovaných metod. Další součásti projektu je prezentace našich výsledků týkajících se potenciální role poruchy regenererace hodnocené pomocí growth-associated protein 43 (GAP43) a role vybraných pro- a anti-inflamatorních cytokinů v patofyziologii DM2 a jeho komplikací. Naše výsledky potvrzují předpoklad, že neuropatie asociovaná s časnými stádii poruch glukózového metabolizmu postihuje převážně senzitivní tenká nervová vlákna, což by mělo být zohledněno při volbě vhodných diagnostických metod u těchto pacientů. Naše výsledky tudíž podporují zahrnutí validního průkazu léze tenkých nervových vláken (např. redukce IENFD) jakožto alternativní kritérium pro diagnózu DSPN vedle kondukčních studií, což je součástí recentních diagnostických kritérií pro diabetickou neuropatii. Naše výsledky potvrzují prediabetes stejně jako časná stádia DM2 jako signifikantní rizikové faktory periferní neuropatie. V naší studii se neuropatie u prediabetu vyskytuje s nižší frekvencí a menší tíží ve srovnání s časnými stádii DM2, což je ve shodě s již publikovanými pracemi. Důležitým klinickým aspektem je možnost efektivní intervence a zlepšení neuropatie u pacientů s poruchou glukózové tolerance, což podtrhuje význam její časné diagnostiky. Výsledky naší další studie zdůrazňují roli porušené regenerace periferních nervů v patofyziologii neuropatie u DM2 a představují growth-associated protein 43 (GAP43) jako nový perspektivní marker incipientní diabetické neuropatie v kožní biopsii. Právě porucha schopnosti regenerace může být limitující v kýženém zlepšení periferní neuropatie u DM2 pacientů a může být tudíž potenciálním terapeutickým cílem. Naše výsledky týkající se systémové i lokální genové exprese vybraných cytokinů a chemokinů zůstávají kontroverzní a je třeba potvrzení dalšími studiemi. Jako významný se jeví nález redukované lokální genové exprese IGF-1 v distálním vzorku kožní biopsie u pacientů s recentním DM2 či prediabetem ve srovnání se zdravými dobrovolníky. Abstract:

Type 2 diabetes mellitus is a chronic disease with increasing prevalence in recent years. Presented set of studies provides a comprehensive prospective assessment of a cohort of patients with early type 2 diabetes mellitus (DM2) and prediabetic state and its comparison with a cohort of healthy normoglycaemic controls. The most beneficial aspect of this study is a detailed investigation of a well-defined group of patients with all the available clinical and laboratory tests used in the diagnostics of peripheral neuropathy. Beside detailed clinical investigation all subjects also underwent nerve conduction studies and electromyography, somatosensory evoked potentials (to exclude the potential coincidence with subclinical central lesion of somatosensory system), the quantitative sensory testing, the spectral analysis of heart rate variability and, importantly, the skin biopsy with evaluation of the intraepidermal nerve fiber density using the panaxonal marker protein gene product 9.5 (PGP9.5) as the most valid skin biopsy parameter. As a part of the complex evaluation of the data acquired from healthy volunteers our own valid and age-stratified normal limits for particular diagnostic methods used (the skin biopsy, the quantitative sensory testing, the spectral analysis of heart rate variability) were also established. In accordance with previously published data, the significant effect of age and a trend towards the impact of gender in normal subjects was observed in all investigated methods. We also present our results regarding the potential role of impaired regeneration assessed by growth-associated protein 43 (GAP43) as well as the role of pro- and anti-inflammatory cytokines in the pathophysiology of DM2 and its complications. Our results confirm the assumption that neuropathy associated with early impaired glucose metabolism predominantly involves sensory small nerve fibers. This fact should be taken into consideration in the choice of appropriate diagnostic methods in these patients. Our results thus support the concept of inclusion of SFN markers (such as the reduction of intraepidermal nerve fiber density) as an alternative criterion for DSPN diagnosis in addition to nerve conduction studies, as proposed in the recent diagnostic criteria for diabetic sensory-motor distal polyneuropathy and diabetic small fiber neuropathy. Our findings also confirm prediabetes and early DM2 as significant risk factors for peripheral neuropathy. Prediabetes-associated neuropathy is less frequent and less serious in comparison with early DM2-associated neuropathy in our study, which is in concordance with the published results. As possible reversal of neuropathy in early stages of glucose dysmetabolism was shown recently, the early and appropriate detection of neuropathic involvement becomes a factor of tremendous importance. The results of our next study emphasize the role of impaired regeneration of peripheral nerves in the pathophysiology of neuropathy in type 2 diabetes mellitus and offer the growth-associated protein 43 as a new perspective marker of incipient diabetic neuropathy in the skin biopsy. The impaired regeneration might prevent the diabetic patients from improvement of peripheral nerve function and might thus serve as a potential therapeutic target. Our results regarding systemic as well as local gene expression of selected cytokines and chemokines remain controversial and need further confirmation. Reduction of local gene expression of insulin-like growth factor I (IGF-I) from the distal skin biopsy sample of patients with early DM2 or prediabetes in comparison with healthy individuals seems to be relevant. The question is whether the metabolic changes induced by hyperglycaemia lead to dysregulation of cytokines or reversely.
