Fibroblast growth factor 23 (FGF23) and its benefit for oncological practice.

Abstract:

Tato bakalářská práce zabývá novým nádorovým markerem Fibroblastovým růstovým faktorem (FGF 23) a jeho možným uplatněním v diagnostice kolorektálního karcinomu a jeho jaterních metastáz. Cílem bylo zavedení metody CLIA (Sandwich Chemiluminescent Immunoassay) na přístroji LIASON XL firmy DiaSorin pro stanovení FGF 23, provést stanovení markeru FGF 23 u probandů normální populace pro získání normálních referenčních hodnot a stanovení u pacientů s kolorektálním karcinomem a pacientů s jaterními metastázami kolorektálního karcinomu. Vyhodnocení zahrnovalo porovnání hladin v jednotlivých skupinách a porovnání s již zavedenými markery, CA 19-9, TPA a TPS jejichž hladiny byly stanoveny u stejných skupin. Po zavedení stanovení FGF 23 metodou CLIA byly referenční hodnoty FGF 23 pro vzorek zdravé populace stanoveny na 30 105 pg/mL. Hodnoty FGF 23 pacientů s kolorektálním karcinomem stadia IV a metastázami do jater před operací a po operaci ukázaly statisticky signifikantní pokles. Hodnoty FGF 23 s žádným jiným stanovovaným markerem nekorelovaly. FGF 23 podle našich výsledků zatím není vhodný jako nádorový marker pro klinickou praxi, jeho stanovení je možno využít pro studium etiopatogeneze nádorového onemocnění.

Abstract:

This bachelor thesis is focused on the new tumor marker Fibroblast Growth Factor 23 (FGF 23) and its possible application in the diagnosis of colorectal carcinoma and its liver metastases. The goal of this thesis was to introduce the new automated chemiluminescent immunoassay (CLIA) method on the DiaSorin LIASON XL instrument to the determination of FGF 23. The main purpose of the FGF 23 introducing was to assess the level of FGF 23 in the normal population to obtain reference values and to assess level of FGF 23 in colorectal cancer patients and patients with the colorectal origin liver metastases. The evaluation included the comparison of the levels in the individual groups and comparison with already established tumor markers CEA, CA 19-9, TPA and TPS whose levels were determined in the same groups. According to the results of the FGF 23 assay by the CLIA method, FGF 23 reference values for the healthy population were set up at 30-105 pg/mL. In patients with colorectal cancer, the level of FGF23 vary with the stage of the disease. In early stages is the level higher than in advanced stages with the liver metastases. The levels of FGF 23 in patients with the stage IV of colorectal cancer before surgery were statistically significantly higher than the levels after surgery. FGF 23 levels did not correlate with any other marker. FGF 23 is not yet usable as a tumor marker in clinical practice, its use can be used for the study of tumor disease etiopathogenesis.
