Respiratory physiotherapy in a client with cystic fibrosis

Abstract:

Cíl: Bakalářská práce si klade za cíl přispět k řešení problematiky při volbě vhodných fyzioterapeutických metod, postupů a konceptů u klienta s cystickou fibrózou v období akutní exacerbace. Metodika: Pro mou klientku jsem zvolila krátkodobý fyzioterapeutický plán, který obsahoval 14 terapií v období její hospitalizace. Z metod jsem použila myofasciální techniky a mobilizace dle Lewita pro ošetření kloubů a měkkých tkání páteře a hrudníku. Dále jsem volila prvky metodiky respirační fyzioterapie pro clearence dýchacích cest, snížení dušnosti a zlepšení statických a dynamických objemů plic. Výsledky: V krátkodobém fyzioterapeutickém plánu jsme dospěly ke zlepšení dušnosti a vylepšení hodnot spirometrie FEV1 z 71 % na 72,4 % , PEF 4,05 l/s na 4,16 l/s a PIF z 2,70 l/s na 2,85 l/s. ABT a pravidelná inhalace pomohly naředit sputum a usnadnila se jeho expektorace. Propojení respirační a klasické fyzioterapie přivedlo pozitivní změny na pohybovém aparátu. Zvětšila se hrudní amplituda ve všech měřených úsecích, pohyblivost páteře a uvolnění hypertonu některých ošetřovaných svalů. Technikou PFS a klasickým strečinkem jsme protáhly nejvíce zkrácené svaly.

Abstract:

Aim: My bachelor thesis aims at contributing to the solution of problems in the choice of suitable physiotherapeutic methods, procedures and concepts in the client with cystic fibrosis in the period of acute exacerbation. Methodology: For my client, I chose a short-term physiotherapy plan that included 14 therapies during the hospitalization period. From the method I used myofascial techniques and Lewit mobilization for the treatment of joints and soft tissues of sleep and chest. Furthermore, I chose the elements of respiratory physiotherapy methodology for respiratory deprivation, reduction of dyspnea and improvement of static and dynamic lung volumes. Results: In short-term physiotherapy we have improved dyspnoea and improved FEV1 spirometry values from 71 % to 72.4 %, PEF 4.05 l/s to 4.16 l/s and PIF from 2.70 l/s to 2.85 l/s. ABT and regular inhalation helped to dilute sputum and facilitate his expectoration. The combination of respiratory and classical physiotherapy has brought positive changes to the musculoskeletal system. The thoracic amplitude has increased in all of the measured sections, the spinal mobility and hypertonic release of some treated muscles. With PFS and classic stretching, we stretched the shortest muscles.
