Detection of large genomic deletions and duplications in BRCA1 gene among woman with hereditary predisposition to breast and ovarian cancer

Abstract:

Syndrom hereditárního karcinomu prsu a ovaria patří mezi nejčastější dědičná onemocnění nejen v české populaci. Většina případů je zapříčiněna zárodečnou mutací v genech BRCA1 nebo BRCA2. Vzhledem k přítomnosti rozsáhlých intragenových delecí v genu BRCA1, které nelze zachytit screeningovými metodami vycházející z PCR reakce, je důležité využívat metodu MLPA v rutinní diagnostice genu BRCA1 u pacientek s dědičnou predispozicí k nádoru prsu a ovarií. Tato metoda založená na principu hybridizace definovaného množství prób a následné multiplex amplifikace pouze u ligovaných prób umožňuje po provedení fragmentační analýzy semikvantitativní detekci produktu, která na základě snížení/zvýšení množství signálu u jednotlivých fragmentů umožňuje identifikovat část genu postiženou delecí nebo duplikací jednoho nebo více exonů. Od začátku zavedení této metody v roce 2005 do března 2014 bylo detekováno u 72 nepříbuzných rodin 12 různých intragenových přeskupení v genu BRCA1: deset různě rozsáhlých intragenových delecí a dvě různé duplikace, které jsou příčinou dědičné predispozice ke vzniku karcinomu prsu a ovaria.

Abstract:

Hereditary breast and ovarian cancer syndrome is one of the most common hereditary disease not only in the czech population. Germline mutations in BRCA1 and BRCA2 genes are responsible for majority of this cases. Owing to presence of large intragenomic deletions in BRCA1 gene, which are not detectable by PCR-based screening methods, it is necessary to use MLPA in routine diagnostics of BRCA1 gene among woman with hereditary predisposition to breast and ovarian cancer. This method is based on hybridisation, ligation, multiplex amplification and subsequent fragment analysis of definite number of probes. MLPA is a semiquantitive method. On the basis of decreased/increased signal of individual fragments this method allows identification of gene part with deletion or duplication of one or more exons. This method has been introduced in our laboratory in 2005. 12 genomic rearrangements have been detected in BRCA1 gene among 72 unrelated families from 2005 to March 2014. In our cohort, we identified 10 different large genomic deletions and 2 different large genomic duplications which cause hereditary predisposition to breast and ovarian cancer.
