Study of new biochemical markers in heart failure

Abstract:

V diagnostice akutního i chronického srdečního selhání je používána řada biochemických markerů. Práce byla zaměřena na dva netradiční kardiální markery – asymetrický dimethylarginin a na inosin. Asymetrický dimethylarginin je posttranslačně modifikovanou formou argininu vytvářenou při procesu degradace methylovaných proteinů a je kompetitivním inhibitorem NO-synthasy. Zvýšená koncentrace asymetrického dimethylargininu v krvi je nezávislým rizikovým faktorem endoteliální dysfunkce a byla pozorována např. u chronického renálního selhání, chronického srdečního selhání, hypertenze, hypercholesterolemie nebo u pacientů s diabetem. Byla vypracována citlivá a reprodukovatelná metoda stanovení L-argininu a také asymetrického a symetrického dimethylargininu v lidské plazmě. Srovnání s publikovanými metodami pak prokázalo použitelnost metody pro stanovení v praxi. Analyty byly stanoveny u souboru pacientů po akutním infarktu myokardu a u souboru diabetických pacientů s nefropatií. Dalším studovaným biochemickým makrem byl inosin. Inosin je purinový nukleosid přirozeně se vyskytující v těle jako součást degradace v metabolismu purinů. Plazmatický inosin je rychle metabolizován v erytrocytech, a proto se fyziologicky vyskytuje v plazmě ve stopovém množství. Inosin je jedním z potenciálních biomarkerů počínající ischemie myokardu. Byla vypracována metoda stanovení adenosinu, inosinu a hypoxanthinu v plazmě. Nově vyvinutá metoda byla použita na stanovení pilotního souboru pacientů po akutním infarktu myokardu.

Abstract:

For diagnosis of acute and chronic heart failure are used many different biochemical markers. Our work was focused on the non-traditional cardiac markers – asymmetric dimethylarginine and inosine. Asymmetric dimethylarginine is posttranslationally modified form of arginine produced in degradation of methylated proteins. Functionally, asymmetric dimethylarginine is a competitive inhibitor of NO-synthase. Increased concentration of asymmetric dimethylarginine in blood is independent risk factor of endothelial dysfunction and it was observed in chronic kidney disease, chronic heart failure, hypertension, hypercholesterolemia and diabetes. We developed sensitive and reproducible method for measurement of L-arginine and also asymmetric and symmetric dimethylarginine in human plasma. Comparison with other methods then showed availability of this method for measuring in clinical practice. Analytes were measured in a group of patients after acute myocardial infarction and in a group of patients with diabetes and nephropathy. Another biochemical marker in our work was inosine. Inosine is purine nucleoside naturally occurring in body as semi-product in purine metabolism. Plasmatic inosine is quickly metabolized in erythrocytes and this is the reason why inosine occurs in plasma in very low concentrations. Inosine is potential biomarker of developing myocardial ischemia. We developed method for measurement of adenosine, inosine and hypoxanthine in human plasma. This newly developed method was used for measurement of group of patients with acute myocardial infarction.
