Coordination of health and social care for patients with AD in the Czech Republic / Slovakia in the context of bed capacities

Abstract:

Alzheimerova choroba je v současné době velmi aktuálním tématem, protože s prodlužujícím se věkem dožití se vyskytuje stále častěji. Nejen osoby s Alzheimerovou chorobou potřebují péči zdravotní i péči sociální. Tuto péči představují dva oddělené resorty, které se jejich zástupci snaží účelně propojit již řadu let a vytvořit tak integrovanou péči zdravotních a sociálních služeb. Účelem této práce je zmapovat péči o pacienty s Alzheimerovou chorobou v lůžkových zařízeních a koordinaci mezi zdravotním a sociálním systémem v rámci ukončení pobytu pacienta s Alzheimerovou chorobou. K tomuto mapování posloužilo dotazníkové šetření, při kterém byli tázáni představitelé nemocnic a LDN v Čechách a na Slovensku. Dotazníky také mapovaly míru výskytu pacientů s AD v nemocnicích/LDN. Dále bylo provedeno několik polostrukturovaných rozhovorů s vybranými zaměstnanci nemocnic a LDN v Čechách a na Slovensku. Rozhovory zjišťovaly několik informací týkajících se příjmu a hospitalizace pacientů s AD, propuštění pacienta s AD a nabídku služeb, které mohou pacienti s AD využít.

Abstract:

Alzheimer's disease is currently a very topical issue, as it occurs more and more with increasing life expectancy. Not only people with Alzheimer's disease need health care but also social care. This care is represented by two separate ministries, which their representatives have been trying to effectively connect for many years and thus create integrated care for health and social services. The purpose of this work is to map care of patients with Alzheimer's disease in inpatient facilities and coordination between the health and social system within the termination of the stay of a patient with Alzheimer's disease. A questionnaire survey was used for this mapping, during which representatives of hospitals and LDN in the Czech Republic and Slovakia were interviewed. The questionnaires also mapped the incidence of patients with AD in hospitals / LDN. Furthermore, several semi-structured interviews were conducted with selected employees of hospitals and LDN in the Czech Republic and Slovakia. The interviews sought some information about income and hospitalization of AD patients, discharge of an AD patient, and a range of services that AD patients can use.
