Abstract:

Úvod: Léčba středně těžké a těžké obstrukční spánkové apnoe (OSA) přetlakovou ventilací je v současnosti považována za "zlatý standard". Při použití přetlakové ventilace je vycházeno z předpokladu, že přetlak adekvátně vyřeší obstrukce na všech úrovních dýchacích cest. Léčba však není v některých případech účinná. Cílem práce bylo zhodnotit využití léky navozené spánkové endoskopie (DISE) a posoudit, zda lze pomocí DISE predikovat efekt léčby přetlakové ventilace a odhalit důvody její neúčinnosti. Metody: Prospektivní studie byla provedena od června 2018 do června 2023. Do studie byli zařazeni pacienti se středně těžkou a těžkou OSA, u kterých byla provedena DISE se současnou zkouškou přetlakové ventilace. Bylo hodnoceno, jak se rozdílný tlak přetlakové ventilace odráží na chování horních cest dýchacích, na jejich expanzi či kolapsu. Sledování efektu bylo provedeno při tlacích 6-18 hPa. U části pacientů byla DISE provedena ve dvou různých polohách (supinační poloha, poloha na boku). Nálezy byly hodnoceny pomocí klasifikace VOTE dle Keziriana, vzájemně porovnány a statisticky analyzovány. U pacientů s nálezem kolabující epiglotis při DISE, při současně neúčinné léčbě přetlakem nebo změnou polohy, byla provedena transorální epiglotopexe. Efekt epiglotopexe byl hodnocen kontrolní limitovanou polygrafií a mezinárodním dotazníkem Epworthskou škálou spavosti. Výsledky: Do studie bylo zařazeno 65 pacientů. Všichni pacienti podstoupili DISE s přetlakovou ventilací a u 44 pacientů bylo provedeno DISE ve dvou různých polohách. Nejlepší efekt polohové terapie byl prokázán na oblast kořene jazyka, kde bylo pozorováno vymizení obstrukce v 27/30 (90 %) případech. Naproti tomu vymizení obstrukce v oblasti měkkého patra bylo zaznamenáno nejméně často, pouze ve 18/42 (43 %) případech. Přetlaková ventilace byla nejméně účinná u kolabující epiglotis, a to pouze v 5/17 (29 %) případech. Ostatní oblasti horních cest dýchacích se mezi sebou významně nelišily a byl u nich pozorován významně lepší efekt. Epiglotopexi podstoupilo 10 pacientů s nálezem kolabující epiglotis. K vyléčení bez potřeby další terapie došlo u 9/10 (90 %) pacientů. U jednoho pacienta došlo pooperačně k téměř 40% poklesu apnoe-hypopnoe indexu s následnou dobrou tolerancí přetlakové ventilace. Pooperační krvácení bylo pozorováno u jednoho pacienta. Závěr: Polohováním pacienta a použitím přetlakové ventilace při DISE lze přímým pohledem na horní dýchací cesty ve spánku predikovat efekt polohové terapie nebo léčby přetlakovou ventilací. Vyšetření umožňuje hodnotit, jak jednotlivé oblasti horních cest dýchacích reagují na léčbu, což lze využít při volbě správné léčby OSA. Epiglotopexe představuje jednoduché a elegantní řešení OSA u pacientů s kolabující epiglotis. Epiglotopexi lze provádět i před zahájením léčby přetlakovou ventilací s cílem zlepšit její toleranci.

Abstract:

Background: Treatment of moderate and severe obstructive sleep apnea (OSA) with positive airway pressure ventilation is currently considered the "gold standard," but in some cases, it is not effective. The application of this treatment is based on the assumption that overpressure will adequately resolve obstructions at all levels of the upper airways. However, overpressure treatment is not effective in some cases. This study aimed to evaluate drug-induced sleep endoscopy (DISE) to assess whether DISE can predict the effect of positive airway pressure and positional therapy and to reveal possible reasons for the ineffectiveness of positive pressure ventilation. Methods: The prospective study was conducted from June 2018 to June 2023. Patients with moderate and severe OSA were enrolled in the study. They underwent DISE with simultaneous positive airway pressure ventilation. The effect of overpressure on upper airways (expansion or collapse) was assessed. The effect was monitored at a pressure of 6- 18 hPa. In some patients, DISE was performed in two positions (supine and lateral positions). The findings were evaluated using the Kezirian VOTE classification, compared with each other, and statistically analyzed. Transoral epiglottopexy was performed in patients with epiglottic collapse found during DISE, with simultaneously ineffective overpressure treatment. The effect of the epiglottopexy was assessed by control limited polygraphy and the international questionnaire Epworth sleepiness scale. Results: 65 patients were included in the study. All patients underwent DISE with positive airway pressure, and 44 also underwent DISE in two different positions. The best effect of positional therapy was demonstrated on the obstruction of the base of the tongue, where the disappearance of the obstruction was observed in 27/30 (90%) cases. In contrast, the disappearance of the obstruction in the area of the soft palate was recorded the least often, only in 18/42 (43%) cases. Positive airway pressure ventilation was the least effective in an epiglottic collapse in only 5/17 (29%) cases. The other areas of the upper airways were not significantly different, and a significantly better effect was observed in them. Ten patients with epiglottic collapse underwent epiglottopexy. Cure without further therapy occurred in 9/10 (90%) patients. In one patient who has not tolerated previous positive airway pressure, there was an almost 40% decrease in the apnea-hypopnea index postoperatively with subsequent good positive airway pressure tolerance. Postoperative bleeding was observed in one patient. Conclusions: By positioning the patient and using positive airway pressure ventilation during DISE, the effect of positional therapy or treatment with positive airway pressure can be predicted. The examination makes it possible to evaluate which areas of the upper airways respond to treatment. These findings can be used when deciding on the appropriate therapy for OSA. Epiglottopexy represents a simple and elegant solution for OSA in patients with epiglottic collapse. Epiglottopexy can also be performed before starting treatment with positive airway pressure to improve tolerance.
