Prediktivní význam metylace promotorové oblasti genu pro MGMT u pacientů s multiformním glioblastomem

Predictive significance of MGMT promoter methylation in glioblastoma multiforme patients

Abstract:

Multiformní glioblastom je nejčastější a zároveň nejzhoubnější primární mozkový nádor dospělých. Medián přežití se pohybuje od 12 do 15 měsíců od stanovení diagnózy. Je to jeden z nejagresivnějších lidských nádorů. Epigenetické utlumení MGMT (O6-methylguanin-DNA methyltransferasa), DNA reparačního proteinu, bylo spojeno s delším přežitím u pacientů s multiformním glioblastomem, kteří byli léčeni radioterapií a konkomitantní chemoterapií s alkylačním činidlem temozolomidem. Testovali jsme vztah mezi methylací promotoru genu pro MGMT v nádorech a přežitím pacientů léčených pomocí temozolomidu. Methylační stav promotoru genu pro MGMT byl určen pomoci bisulfitové modifikace a methylačně specifické PCR. Dále jsme srovnávali, zda je methylace promotoru genu pro MGMT zachována i v primární kultuře z resekátu glioblastomu. Celková nebo parciální methylace promotoru genu pro MGMT byla pozorována u 5 ze 7 zkoumaných vzorků tkáně a u 4 ze 7 vzorků primárních kultur. Až na jeden vzorek glioblastomu, byl methylační stav promotoru genu pro MGMT během kultivace explantátové primární kultury zachován. Stanovena methylace promotoru genu pro MGMT byla srovnána s údaji o přežití pacientů. I když pacienti s methylovaným promotorem mají relativně dlouhý čas do progrese, dlouhého času do progrese dosáhl i jeden pacient s nemethylovaným promotorem. Z naší studie vyplývá, že primární kultury glioblastomu se jeví jako vhodný model pro studium chemorezistence glioblastomových buněk k alkylačním činidlům. Výsledky naší studie nepodporují význam methylace promotoru genu pro MGMT jako klíčového prediktivního faktoru odpovědi na léčbu (radioterapii kombinovanou s konkomitantní nebo adjuvantní terapií temozolomidem) u pacientů s mutiformním glioblastomem.

Abstract:

Glioblastoma multiforme (GBM ) is the most common primary brain tumor in adults and also the most malignit tumor of glial origin. It is one of the most aggressive human neoplasms, with a median survival ranging from 12 to 15 months. Epigenetic silencing of the MGMT (O6-methylguanine-DNA methyltransferase) by its promotor methylation compromises DNA repair and has been associated with longer survival in patients with glioblastoma who receive alkylating agents. We tested the relationship between MGMT silencing in the tumor and the survival of patients treated with radiotherapy and concomitant chemotherapy with alkylating agent temozolomide. The methylation status of the MGMT promoter was determined by methylation-specific polymerase-chain-reaction analysis. We examined glioblastomas to determine whether methylation status of the MGMT promoter in glioblastoma tissue is conserved also in derived primary culture. The MGMT promoter was methylated in 5 of the 7 tissue samples involved and in 4 of the 7 samples of primary cultures. Out of one sample, the MGMT promoter methylation status remained stable during cultivation of primary cell lines from glioblastoma specimens. The MGMT promoter methylation status was compared with the survival of patients. Although patients with presence of the methylated MGMT promoter tended to have longer time to progression, one patient with completely unmethylated MGMT promoter achieved relatively long time to progression. Our study shows that the primary glioblastoma cultures seem as an feasible model for studying tumor chemoresistance to alkylating agents because methylation status remains conserved during thein cultivation. According to the result of our study, we can not support the importance of promoter methylation of the MGMT gene as a key predictive factor of response to therapy (radiotherapy combined with concomitant and adjutant treatment with temozolomide) in patients with glioblastoma multiforme.
