Bezinterferonová léčba chronické hepatitidy C a její vliv na hladiny mikropartikulí a e-selektinu

Interferon-free treatment of chronic hepatitis C and its effect on microparticles and e-selectin levels

Abstract:

Úvod. Chronická hepatitida C (CHC) představuje významný globální zdravotní problém. Po zavedení přímo působících antivirotik (DAA) do klinické praxe došlo k rapidnímu zlepšení efektivity terapie. Při použití druhé generace DAA dosáhne setrvalé virologické odpovědi (SVR) 98-100 % léčených. CHC je charakteristická přítomností chronického zánětu jater, který přetrvává i po dosažení SVR. Cílem práce bylo zhodnotit epidemiologické charakteristiky současné populace pacientů s CHC, efekt dosažení SVR na jaterní funkce a strukturu a posouzení vývoje zánětlivých parametrů včetně potenciálních nových biomarkerů zánětu: mikropartikulí (MP) a e-selektinu (ES). Metody. Prospektivní studie pacientů s CHC léčených pomocí DAA. U pacientů byla v rámci kontrol na začátku léčby (K1) na konci léčby DAA (K2) a 12 týdnů po konci léčby (K3) hodnocena virémie, laboratorní ukazatele jaterních funkcí a poškození, tuhost/elasticita jater pomocí transientní elastografie (TE). Byly stanovovány i hladiny MP a ES, které byly porovnány s kontrolní skupinou 40 zdravých dobrovolníků. Výsledky. Celkem 36 pacientů léčených pomocí DAA bylo zařazeno do studie. U všech bylo dosaženo SVR. Dosažení SVR bylo spjaté se signifikantní zlepšením laboratorních ukazatelů jaterních funkcí i poškození. Hodnoty TE mezi K1 a K3 se významně snížily (mediány 6,3 kPa; 5,3 kPa; p=0,002). Hladiny MP a ES byly signifikantně vyšší než u kontrolní skupiny na začátku léčby i na konci sledování (v K1 i K3; p<0,001 pro MP i ES). Pokles koncentrace MP mezi K1 a K3 nebyl statisticky významný (p=0,330), u ES významným byl (p<0,001). V běžné praxi užívané parametry zánětu jako sedimentace erytrocytů, C-reaktivní protein či koncentrace leukocytů nebyly alterovány a významně se během sledování neměnily. Závěr. Terapie CHC pomocí DAA je bezpečná a efektivní. Hladiny MP a ES jsou u pacientů s CHC významně zvýšené a po dosažení SVR dochází k jejich poklesu. Vyšší koncentrace proti kontrolní skupině na konci sledování naznačují perzistenci zánětlivé reakce v jaterním parenchymu i po dosažení SVR. MP a ES jsou navíc senzitivnější než rutinně užívané markery zánětu.

Abstract:

Introduction. Chronic hepatitis C (CHC) is a major global health problem. The introduction of direct-acting antivirotics (DAA) into the clinical praxis significantly improved the success rate of CHC therapy. Between 98 to 100 % of patients treated with second-generation DAA achieve sustained virological response (SVR). Chronic liver inflammation is characteristic of CHC. The aims of this study were to determine an epidemiological characteristic of the current CHC population, evaluate the effect of SVR on liver function and structure, and analyse of old and new inflammation biomarkers: microparticles (MP) and e-selectin in patients with CHC. Methods. A prospective study of patients with CHC treated with DAA. A viremia, laboratory markers of liver function and damage, transient elastography (TE), and levels of MP and ES were assessed at study visits at the beginning of DAA treatment (K1), at the end of therapy (K2), and 12 weeks after the end of treatment. A group of 40 healthy volunteers was used as a control group (CG). Results. A total of 36 patients treated with DAA was included in the study. All patients achieved SVR. The SVR was associated with significant improvement of laboratory markers of liver function and damage. Liver stiffness measured by TE reduced during follow-up (median 6.3 kPa at K1, 5.3 kPa at K3, p=0.002). Levels of MP and ES were significantly higher in the study group than in CG both at K1 (p<0.001) and K3 (p<0.001). A statistically significant decrease in MP levels was not observed between K1 and K3 (p=0.330), contrary ES levels declined significantly (p<0.001). Routine inflammatory markers like erythrocyte sedimentary rate (ESR), white blood count (WBC), or C-reactive protein (CRP) were not elevated and did not change during study. Conclusions. DAA therapy of CHC was proved to be safe and effective. The levels of MP and ES are significantly higher in patients with CHC and decrease after the end of DAA treatment. Their higher concentration in the study group 12 weeks after the end of DAA therapy implies the persistence of inflammation in liver tissue even after achieving SVR. MP and ES are more sensitive markers of inflammation than routinely used ESR, WBC, and CRP.
