Measure of depression and a quality of seniors' life in a practical doctor's outpatients' department

Abstract:

Cíl: Práce se zabývá problematikou kvality života a depresivním syndromem ve stáří. Cílem diplomové práce bylo posoudit kvalitu života seniora s depresí a bez deprese, zjistit vztahy mezi mírou deprese a kvalitou života seniorů v ambulanci praktického lékaře. Výzkum zjišťoval rozdíly v posuzovaných parametrech v závislosti na míře deprese, identifikoval změny v kvalitě života a životní spokojenosti seniorů s depresí. Metodika: Pro kvantitativní výzkum byla zvolena metoda sběru dat formou baterie 4 standardizovaných instrumentů (MMSE, GDS, WHOQOL?BREF, WHOQOL?OLD) doplněných otázkami zjišťující sociodemografické údaje. Do souboru byli zařazeni všichni respondenti ve věku 65 a více let, MMSE nad 18 bodů, kteří navštívili v rozhodném období ambulanci praktického lékaře. Výzkumný soubor tvořil 166 respondentů. Výsledky: Z výsledků našeho výzkumu vyplývá, že z celkového počtu respondentů byla zjištěna mírná deprese u 16 (10 %) seniorů a manifestní u 14 (8 %). Výskyt deprese se mezi muži a ženami v seniorské populaci lišil (p = 0,014). Při statistickém zpracování míry deprese vs. pohlaví, věk, vzdělání, rodinný stav a typ bydlení nebyla prokázána závislost. U respondentů se významně statisticky lišila kvalita života seniorů s depresí a bez deprese (p < 0,001). Při hodnocení kvality života seniorů s mírnou depresí a s manifestní byl sledován významný statistický rozdíl v jedné položce a pěti doménách instrumentů WHOQOL?BREF a WHOQOL-OLD. Závěr: Deprese je jeden z důležitých faktorů ovlivňující kvalitu života seniorů. Při aktivním vyhledávání depresivních symptomů a léčbě zvyšujeme seniorům kvalitu prožitých let.

Abstract:

Goal: This thesis deals with the issue of quality of life with depressive syndrome in old age. The goal of the thesis was to assess quality of life by seniors with depression and without depression, find out relations between level of depression and quality of life by seniors in ambulance of general practitioner. The research monitored differences between selected factors depending on the level of depression and identified changes in quality of life and life satisfaction of seniors with depression. Methodology: The quantitative research was conducted by method of collecting data with the form of battery of four standardized instruments- MMSE, GDS, WHOQOL?BREF and WHOQOL?OLD. These questionnaires were completed by questions about sociodemographic data. Respondents were 65 years old and older and they visited the ambulance of general practitioner during the relevant period. The criterion of inclusion in the research was level of MMSE (18 and more). Overall, 166 respondents participated in this research. Results: From results of this research we found that 16 seniors (10 %) had a mild depression and 14 seniors (8 %) had a manifest depression. We found a huge statistic difference in occurrence of depression between men and women in senior population (p = 0,014). We didn?t prove that level of depression depends of gender, age, education, marital status or type of housing. The quality of life by seniors with depression and without depression was statically very different (p < 0,001). We found important statistical difference in one item and five domain in WHOQOL-BREF instrument and in WHOQOL-OLD instrument during the rating of quality of life by seniors with mild depression and manifest depression. Conclusion: Depression is one of the main factor affecting the quality of life of seniors. We should actively search for seniors with depressive symptoms and treatment, we ensure improve their quality of life and relive years.
