Genetic diagnostics in childhood thrombocytopenia using whole genome optical mapping

Abstract:

Trombocytopenie (TP) jsou charakterizovány sníženym počtem trombocytů z důvodu nerovnováhy mezi tvorbou a zánikem trombocytů v periferní krvi. TP je často spojována s mutacemi v genech, které se podílejí na diferenciaci megakaryocytů a/nebo tvorbě trombocytů. Navzdory pokrokům v diagnostice pomocí sekvenování nové generace zůstávají u mnoha případů molekulární mechanismy a příčiny vzniku TP neobjasněny. V teoretické části práce byla provedena literární rešerše na témata patogeneze TP u dětí, její příčiny a molekulární podstata a možnosti sekvenování nové generace (NGS) a celogenomového optického mapování (OGM) k analyze genetickych aberací. V experimentální části byla využita metoda OGM ke genetické analyze u dětskych pacientů s TP. OGM je inovativní metoda schopná detekovat strukturní varianty (SVs) jako jsou velké delece, inzerce, duplikace nebo inverze, které není možno detekovat pomocí tradičním NGS s krátkym čtením. Z periferní krve byla vyizolovaná dlouhá vysokomolekulární DNA, která se následně fluorescenčně označila a po nanesení na čip se vizualizovala na přístroji Saphyr (Bionano Gernomics). Následovalo bioinformatické sestavení celogenomové mapy a porovnání s referenčním lidskym genomem hg38 a kontrolní databází zdravych jedinců. Vysledky OGM ukázaly velkou heterogenitu nalezenych SVs mezi jednotlivymi pacienty. Nejčastěji detekované SV byly delece, které tvořily více než 50 % všech detekovanych SVs. Dále byly detekovány inzerce (33 %), duplikace (2,7 %) a inverze (8,8 %). U většiny pacientů byly nalezeny SVs zahrnující geny, které mohou hrát roli v patogenezi TP. Příkladem SV je nalezená delece (14,5 kbp) u pacienta P1 zahrnující gen IL4R na chromozomu 16 jehož mutace byla již dříve spojovaná na s TP. U pacienta P2 byla nalezena delece na chromozomu 20 (4,4 kbp) v blízkosti genu CD40 kódujícím klíčovy receptor na antigen prezentujících buňkách. Další nalezené delece na chromozomu 1 (1,6 kbp) a delece na chromozomu 11 (2,3 kbp) zahrnují geny asociované s metabolismem vápníku, ktery hraje důležitou roli v krvetvorbě. Další nalezená delece na chromozomu 2 (14,7 kbp) se nacházela v blízkosti genu VWF, ktery zajišťuje produkci von Willebrandova faktoru iniciujícího adhezi a agregaci trombocytů. Vysledky získané v této diplomové práci poukazují na možnosti využití inovativní genetickych přístupů založenych na analyze dlouhé vysokomolekulární DNA k detekci variant, které nelze detekovat současnymi technikami NGS. Nově nalezené SV by mohly přispět k lepšímu pochopení variability lidského genomu a ovlivnit prognózu u pacientů s TP.

Abstract:

Thrombocytopenias (TP) are characterized by a by a reduced platelet count due to an imbalance between platelet production and death in the peripheral blood. TP is often associated with mutations in genes involved in megakaryocyte differentiation and/or platelet formation. Despite advances diagnostic using next generation sequencing (NGS) the molecular mechanisms and causes of TP remain unclear in many cases. In the theoretical part of the thesis, a literature review was performed on the pathogenesis of TP in children, its causes and molecular basis, and on the possibilities of next-generation sequencing (NGS) and whole-genome optical mapping (OGM) to analyze genetic aberrations. In the experimental part, OGM was used for genetic analysis in pediatric patients with TP. OGM is an innovative method capable of detecting structural variants (SVs) such as large deletions, insertions, duplications or inversions that cannot be detected by traditional short-read NGS. Long high molecular weight DNA was isolated from peripheral blood, fluorescently labelled and visualized on a Saphyr chip (Bionano Gernomics). This was followed by bioinformatic construction of a whole genome map and comparison with the reference human genome hg38 and a control database of healthy individuals. OGM results showed a large heterogeneity of SVs found among patients. The most common SVs were deletions, which accounted for more than 50 % of all detected SVs. Insertions (33 %), duplications (2.7 %) and inversions (8.8 %) were also found. Most patients were found to have SVs involving genes that may play a role in the pathogenesis of TP. An example of SV is the deletion (14.5 kbp) found in patient P1 involving the IL4R gene on chromosome 16, the mutation of which was previously associated with TP. In patient P2, a deletion on chromosome 20 (4.4 kbp) was found near the CD40 gene encoding a key receptor on antigen presenting cells. Other deletions found on chromosome 1 (1.6 kbp) and a deletion on chromosome 11 (2.3 kbp) involve genes associated with calcium metabolism, which plays an important role in hematopoiesis. Another deletion found on chromosome 2 (14.7 kbp) was located near the VWF gene, which is responsible for the production of von Willebrand factor that initiates platelet adhesion and aggregation. The results obtained in this thesis highlight the potential of using innovative genetic approaches based on long high molecular weight DNA analysis to detect variants that cannot be detected by current NGS techniques. The newly discovered SVs could contribute to a better understanding of human genome variation and influence the prognosis of TP patients.
