Diagnosis and therapy of flat foot deformity in childhood

Abstract:

Plochá noha v dětském věku patří k nejčastějším diagnózám v ambulancích ortopedie nebo fyzioterapie. Cílem této disertační práce je posoudit vliv rehabilitační intervence u dětské flexibilní ploché nohy. Výzkumný soubor tvořilo 20 účastníků (5 dívek a 15 chlapců). Průměrný věk byl 5,8 ± 0,9 let. Měření proběhlo pomocí tradičních 2D metod (dynamická plantografie, podoskop) a nové 3D profilometrické skenovací metody. Měřené parametry byly hodnoty průměrných tlaků, maximálních tlaků a kontaktní plochy chodidla v oblasti středonoží ve stoji. Při chůzi byly v oblasti středonoží hodnoceny průměrné a maximální tlaky a časový integrál tlaku. Posuzovala se míra flexibility nožní klenby a míra pronace nohy. Měřené 3D parametry byly výška nožní klenby bez a v zatížení a longitudinal arch angle. Měření bylo provedeno před a po rehabilitační intervenci. Po intervenci došlo u všech sledovaných proměnných k statisticky významným změnám ve smyslu zvýšení nožní klenby ( p< 0,05). Tato studie popisuje možnost jak diagnostiky, tak i zhodnocení efektu terapie dětské ploché nohy pomocí 2D a 3D metod.

Abstract:

Flatfoot diagnosis in childhood belongs to the most frequent diagnosis in the orthopedic and physiotherapist clinics. The aim of this disertation thesis is to evaluate the influence of rehabilitation on the flatfoot diagnosis in childhood. The research group consisted of 20 participants with the flatfoot diagnosis (5 girls and 15 boys), aged 5.8 ± 0.9 years. The measurement was performed by using traditional 2D methods (dynamic plantography, podoskop) and a new 3D profilometry scanning method. The measured parameters were the mean pressures, peak presssures and contact area in midfoot in stance. The mean pressures, the peak pressures and the pressure-time integral were measured in midfoot during a gait. Then, there were evaluated the flexibility of the foot arch and the pronation of the foot. The measured 3D parameters were: the arch height without loading and in loading and longitudinal arch angle. This measurement was performed before and after rehabilitation intervention. Statistical analysis of data showed statistically significant changes of all parametres after the intervention (p<.05). The foot arch was significantly higher. This study describes the possibility of using 2D methods and 3D scanning method for diagnosis and evaluation of therapy effect in the flatfoot diagnosis in childhood.
